健脾泄浊解毒方治疗糖尿病肾病临床研究

摘　　要：目的:观察健脾泄浊解毒方对糖尿病肾病(DN)患者炎症因子、氧化因子、血糖、肾功能、肠道菌群等指标的影响。方法:采用随机数字法将100例DN患者平均分为对照组和观察组各50例,对照组给予常规疗法治疗,观察组给予健脾泄浊解毒方治疗,比较治疗前后两组患者炎症因子(CRP、TNF-α、IL-6)、血糖(PBG、FBG、HbA1c)、肾功能(UALB、BUN、eGFR)、氧化因子(SOD、MDA、TAOC)、免疫因子(CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+))及肠道菌群(双歧杆菌、乳酸杆菌、拟杆菌、肠球菌、大肠杆菌)的变化。结果:治疗后,观察组患者CRP、TNF-α、IL-6、MDA、CD8^(+)、PBG、FBG、HbA1c水平及肠球菌数目均显著降低,且观察组各水平均显著低于对照组;而两组患者SOD、TAOC、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)比值、eGFR及有益菌群的数量均升高,且观察组的SOD、TAOC、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)比值、eGFR及有益菌群的数值均比对照组显著升高(P<0.05)。结论:健脾泄浊解毒方治疗糖尿病肾病可显著减轻炎症反应,提升免疫功能,改善患者血糖水平、氧化因子水平和肾功能指标,且利于机体肠道有益菌群的增加,疗效显著。Objective:To observe the effects of Jianpi Xiezhuo Jiedu prescription on inflammatory factors,oxidation factors,blood glucose,renal function,intestinal flora and other indexes in diabetic nephropathy(DN)patients.Methods:A total of 100 diabetic nephropathy patients were included,and their scores were divided into two groups by random number method.The control group was treated with conventional therapy,and the observation group was treated with Jianpi Xiuzhu Jiedu prescription.Inflammatory factors(CRP,TNF-α,IL-6),blood glucose(PBG,FBG,HbA1c),renal function(UALB,BUN,eGFR),oxidation factors(SOD,MDA,TAOC),immune factors(CD3^(+),CD4^(+),CD4^(+)/CD8^(+))and intestinal flora(bifidum)were compared between the two groups before and after treatment(Bacillus,Lactobacillus,Bacteroides,Enterococcus,Escherichia coli).Results:After treatment,the levels of CRP,TNF-α,IL-6,MDA,CD8^(+),PBG,FBG,HbA1c and enterococcus in observation group were significantly decreased,and the levels found in the observation group were considerably lower than the ones in the control group.SOD,TAOC,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)ratio,eGFR and the number of bifidobacterium,Moreover,SOD,TAOC,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)ratio,eGFR and beneficial bacteria in observation group were significantly higher than those in control group(P<0.05).Conclusion:Jianpi Xiezhuo Jiedu prescription in the treatment of diabetic nephropathy can not only significantly reduce the body inflammatory response of patients,enhance the immune level,but also help to improve the blood sugar level,oxidation factor level and renal function index of patients,and at the same time,it is conducive to the increase of beneficial bacteria in the intestinal tract of the body,with significant curative effect.
