支气管哮喘患儿血清白三烯B4 骨桥蛋白 分泌型卷曲相关蛋白1水平变化及临床意义分析

摘　　要：目的探讨支气管哮喘患儿血清白三烯B4、骨桥蛋白、分泌型卷曲相关蛋白1(SFRP1)水平变化及临床意义。方法选取2021年1月—2022年5月宁波大学附属人民医院收治的支气管哮喘患儿108例,根据支气管哮喘发作情况分为缓解期组37例,急性发作期组71例;另收集在宁波大学附属人民医院体检的健康儿童30例为健康对照组。收集受试儿童空腹外周静脉血,采用酶联免疫吸附法检测血清白三烯B4、骨桥蛋白、SFRP1水平。对急性发作期支气管哮喘患儿进行6个月随访,末次随访时评价疾病控制水平,分为完全控制组、部分控制组、未控制组,将完全控制和部分控制的患儿定义为预后良好,未控制的患儿定义为预后不良,进行受试者工作特征(ROC)曲线分析。结果急性发作期组血清白三烯B4、骨桥蛋白、SFRP1水平均明显高于缓解期组和健康对照组,缓解期组血清白三烯B4、骨桥蛋白、SFRP1水平均明显高于健康对照组,差异均有统计学意义(均P<0.05)。重度组血清白三烯B4、骨桥蛋白、SFRP1水平均明显高于中度组和轻度组,中度组血清白三烯B4、骨桥蛋白、SFRP1水平均明显高于轻度组,差异均有统计学意义(均P<0.05)。未控制组患儿血清白三烯B4、骨桥蛋白、SFRP1水平[(27.79±5.02)ng/L、(689.50±98.13)ng/ml、(114.78±6.63)pg/ml]均明显高于部分控制组、完全控制组患儿,部分控制组患儿血清白三烯B4、骨桥蛋白、SFRP1水平[(22.09±4.15)ng/L、(627.51±85.37)ng/ml、(101.59±5.65)pg/ml]均明显高于完全控制组患儿,差异均有统计学意义(均P<0.05)。ROC曲线显示:血清白三烯B4、骨桥蛋白、SFRP1截断值分别为20.71 ng/L、594.29 ng/ml、95.06 pg/ml时对急性发作期支气管哮喘患儿预后具有较高的预测价值(P<0.05),其中各指标联合检测的预测价值最高,曲线下面积为0.968。结论支气管哮喘患儿血清白三烯B4、骨桥蛋白、SFRP1水平明显升高,且与�Objective To explore the changes of levels of serum leukotriene B4,osteopontin,and secreted frizzled-related protein 1(SFRP1)in children with bronchial asthma and clinical significance.Methods A total of 108 children with bronchial asthma treated in People's Hospital of Ningbo University from January 2021 to May 2022 were selected and divided into remission period group(37 cases)and acute attack period group(71 cases)according to onset of bronchial asthma.Thirty healthy children receiving physical examination in People's Hospital of Ningbo University were collected as healthy control group.The fasting peripheral venous blood samples were collected,the levels of serum leukotriene B4,osteopontin,and SFRP1 were detected by enzyme-linked immunosorbent assay.The children in acute attack period group were followed up for 6 months,the control level of the disease was evaluated during the last follow-up,then the children were divided into complete control group,partial control group,and non-control group,the children in complete control group and partial control group were designed as good prognosis,and the children in non-control group were designed as bad prognosis,receiver operating characteristic(ROC)curve analysis was performed.Results The levels of serum leukotriene B4,osteopontin,and SFRP1 in acute attack period group were significantly higher than those in remission period group and healthy control group,the levels of serum leukotriene B4,osteopontin,and SFRP1 in remission period group were significantly higher than those in healthy control group(all P<0.05).The levels of serum leukotriene B4,osteopontin,and SFRP1 in mild group were significantly higher than those in moderate group and mild group,the levels of serum leukotriene B4,osteopontin,and SFRP1 in moderate group were significantly higher than those in mild group(all P<0.05).The levels of serum leukotriene B4,osteopontin,and SFRP1 in non-control group[(27.79±5.02)ng/L,(689.50±98.13)ng/ml,(114.78±6.63)pg/ml]were significantly higher than those in complet
