血清SIRT1与syndecan-4在心力衰竭中的表达及临床价值研究

摘　　要：目的分析血清沉默信息调节因子2相关酶1(Silent information regulatory factor 2-related enzyme 1,SIRT1)与人多配体蛋白聚糖4(Human multiligand proteoglycan 4,syndecan-4)在心力衰竭(heart failure,HF)中的表达与临床价值。方法将九江学院附属医院2023年1月1日-2023年12月31日收治的142例HF患者作为本次观察对象,另将同一时期接收的健康志愿者142名作为对照组观察对象,分析两组SIRT1与syndecan-4水平变化,比较不同心功能分级SIRT1与syndecan-4水平表达情况,Logistic回归分析SIRT1、syndecan-4与HF的相关性;根据HF患者治疗后预后不同完成分组,即预后不良组(病情进一步恶化或死亡,20例)与预后良好组(病情好转出院,122例),分析两组SIRT1与syndecan-4水平变化情况。结果HF组SIRT1水平低于对照组,syndecan-4水平高于对照组,差异有统计学意义(P<0.05);美国纽约心脏病协会(American New York Heart Association,NYHA)Ⅰ级、Ⅱ级、Ⅲ级、Ⅳ级SIRT1水平从高到低依次为:Ⅰ级>Ⅱ级>Ⅲ级>Ⅳ级,syndecan-4从高到低顺序依次为:Ⅳ级>Ⅲ级>Ⅱ级>Ⅰ级(P<0.05);预后不良组SIRT1水平低于预后良好组,syndecan-4水平高于预后良好组,差异有统计学意义(P<0.05);Logistic回归分析可见SIRT1与HF呈负相关,syndecan-4与HF呈正相关。结论SIRT1与syndecan-4在HF疾病中有异常表达,有助于临床观察病情严重程度,不仅可作为疾病诊断与病情评估指标,也能用于预测不良预后,与HF有较大关联。Objective Analysis of the expression and clinical value of serum silencing information regulatory factor 2-related enzyme 1(SIRT1)and human multiligand proteoglycan 4(syndecan-4)in heart failure(HF).Methods A total of 142 HF patients admitted to the Affiliated Hospital of Jiujiang University from January 1,2023 to December 31,2023 were selected as the subjects of this study,and 142 healthy volunteers admitted during the same period were selected as the control group.The changes of SIRT1 and syndecan-4 levels in the two groups were analyzed.The expression levels of SIRT1 and syndecan-4 in different cardiac function grades were compared,and the correlation between SIRT1 and syndecan-4 and HF was analyzed by Logistic regression.HF patients were divided into groups according to different prognosis after treatment,that is,the poor prognosis group(further deterioration or death,20 cases)and the good prognosis group(condition improvement and discharge,122 cases).The changes of SIRT1 and syndecan-4 levels in the two groups were analyzed.Results The SIRT1 level in HF group was lower than that in control group,and the syndecan-4 level was higher than that in control group,the difference was statistically significant(P<0.05).American New York Heart Association(NYHA)gradeⅠ,Ⅱ,Ⅲ,ⅣSIRT1 levels from high to low are:GradeⅠ>GradeⅡ>GradeⅢ>GradeⅣ,syndecan-4 from high to low order is:GradeⅣ>GradeⅢ>GradeⅡ>GradeⅠ(P<0.05);the SIRT1 level in poor prognosis group was lower than that in good prognosis group,and the syndecan-4 level was higher than that in good prognosis group,the difference was statistically significant(P<0.05).Logistic regression analysis showed that SIRT1 was negatively correlated with HF,and syndecan-4 was positively correlated with HF.Conclusion SIRT1 and syndecan-4 have abnormal expression in HF diseases,which can gradually decrease with the progression of disease grade,which is helpful for clinical observation of the severity of the disease.It can not only be used as an indicator of disease di
