纤维支气管镜辅助抗感染治疗重症呼吸机相关性肺炎的临床疗效观察

摘　　要：目的观察纤维支气管镜应用在重症呼吸机相关性肺炎患者中的抗感染效果。方法自2021年8月-2022年3月期间随机选取本院收治的重症呼吸机相关性肺炎患者100例,采用随机数字表法对其进行分组,对照组实施雾化吸入阿卡米星治疗,观察组基于此增加纤维支气管镜辅助治疗,对比两组患者的治疗效果、治疗前后呼吸力学各指标。结果观察组患者的临床疗效优于对照组,差异存在统计学意义(P<0.05)。观察组与对照组的呼吸力学各指标对比,治疗前无统计学意义(P>0.05),治疗后,观察组的各项指标比对照组更好(P<0.05)。其中,Raw、PIP、WOB比对照组低,Cdyn指标则高于对照组。结论纤维支气管镜应用在重症呼吸机相关性肺炎患者中,可以提高患者的生活质量,改善患者的呼吸力学指标,恢复患者的呼吸功能,具有显著效果,存在一定的临床推广应用价值。Objective To observe the anti infection effect of fiberoptic bronchoscopy in patients with severe ventilator-associated pneumonia.Methods 100 patients with severe ventilator-associated pneumonia admitted to our hospital were randomly selected from August 2021 to March 2022,the patients were randomly divided into two groups using a digital randomization table method.The control group received nebulized inhalation of acaricin,while the observation group received additional fiberoptic bronchoscopy treatment based on this.The treatment effects and respiratory mechanics indicators before and after treatment were compared between the two groups.Results The clinical efficacy of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in respiratory mechanics indicators between the observation group and the control group before treatment(P>0.05).After treatment,the various indicators of the observation group were better than those of the control group(P<0.05).Among them,Raw,PIP,and WOB were lower than the control group,while Cdyn indicators were higher than the control group.Conclusion The application of fiberoptic bronchoscopy in severe ventilator-associated pneumonia patients can improve their quality of life,improve their respiratory mechanics indicators,and restore their respiratory function.It has significant effects and has certain clinical promotion and application value.
