地屈孕酮片配合宫腔镜电切术治疗对子宫内膜息肉患者子宫内膜厚度、月经量及复发率的影响

摘　　要：目的探究地屈孕酮片联合宫腔镜电切术(TCRP)治疗对子宫内膜息肉(EP)患者子宫内膜厚度(EST)、月经量及复发率的影响。方法共纳入南部战区海军第一医院收治的97例EP患者,时间为2020年1月-2023年12月,以随机数字表法分为两组,即对照组(n=49)、观察组(n=48)。对照组予以TCRP治疗,观察组配合地屈孕酮片治疗,服药3个月;于治疗前、后比较两组月经量、月经周期、EST值、血管内皮生长因子(VEGF)水平;并比较两组随访3个月、6个月时复发率。结果治疗后,两组月经量减少、月经周期延长,且观察组月经量少于对照组,月经周期长于对照组(P<0.05);治疗后,两组EST值、血清VEGF水平下降,且观察组低于对照组(P<0.05)。两组随访3个月时复发率比较,无统计学差异(P>0.05);随访6个月时,观察组复发率低于对照组(P<0.05)。结论地屈孕酮片联合TCRP治疗EP能够减少月经量、延长月经周期、降低EST值、调节血清VEGF水平、降低复发率。Objective To investigate the effects of the combination of progesterone tablets and hysteroscopic resection(TCRP)on endometrial thickness(EST),menstrual flow,and recurrence rate in patients with endometrial polyps(EP).Methods A total of 97 EP patients admitted to the hospital from January 2020 to December 2023 were randomly divided into two groups using a random number table method:a control group(n=49)and an observation group(n=48).The control group was treated with TCRP,while the observation group was treated with dexamethasone tablets for 3 months;compare the menstrual flow,menstrual cycle,EST values,and vascular endothelial growth factor(VEGF)levels between the two groups before and after treatment;and compare the recurrence rates between the two groups at 3 and 6 months of follow-up.Results After treatment,both groups experienced a decrease in menstrual flow and an extension of menstrual cycle,with the observation group having less menstrual flow and longer menstrual cycle than the control group(P<0.05);after treatment,the EST values and serum VEGF levels in both groups decreased,and the observation group was lower than the control group(P<0.05).There was no statistically significant difference(P>0.05)in the recurrence rate between the two groups after a 3-month followup;at the 6-month follow-up,the recurrence rate in the observation group was lower than that in the control group(P<0.05).Conclusion The combination of progesterone tablets and TCRP in the treatment of EP can reduce menstrual flow,prolong menstrual cycles,lower EST values,regulate serum VEGF levels,and reduce recurrence rates.
