达格列净对合并不同程度蛋白尿的2型糖尿病患者血脂水平的影响

摘　　要：目的观察达格列净对于2型糖尿病(T2DM)合并不同程度蛋白尿的患者治疗前后尿白蛋白及血脂水平的变化。方法选取2021年6月-2022年6月在滨州市中心医院内分泌科住院的T2DM患者90例,按照尿白蛋白/肌酐比值(UACR)水平分组,分为正常白蛋白尿组28例;微量白蛋白尿组32例;大量白蛋白尿组30例,全部患者加用达格列净10mg/日,对比治疗前后血脂的变化情况。结果治疗12周后,三组的甘油三酯、总胆固醇、LDL-C水平及UACR水平均有明显下降,HDL-C水平高于治疗前,差异有统计学意义(P<0.05)。结论达格列净对于T2DM合并不同程度蛋白尿的患者的血糖、血脂均可发挥较强的调节与改善作用,同时可缓解病情。Objective To observe the changes in urine albumin and lipid levels before and after treatment with dapagliflozin in patients with type 2 diabetes mellitus(T2DM)combined with varying degrees of proteinuria.Methods Ninety inpatients with T2DM treated in the Endocrinology Department of Binzhou Central Hospital from June 2021 to June 2022 were selected.They were grouped according to urine albumin/creatinine ratio(UACR)levels:28 patients in the normal albuminuria group,32 patients in the microalbuminuria group,and 30 patients in the macroalbuminuria group.All patients received dapagliflozin 10mg/day in addition to their existing treatment regimen.Changes in lipid profiles before and after treatment were compared.Results After 12 weeks of treatment,significant reductions were observed in triglycerides,total cholesterol,LDL-C levels,and UACR levels in all three groups,HDL-C levels increased compared to baseline,with statistically significant differences(P<0.05).Conclusion Dapagliflozin exerts strong regulatory and improvement effects on blood glucose and lipid profiles in patients with T2DM and varying degrees of proteinuria.It also mitigates disease progression.
