摘　　要：目的分析北京市顺义区马坡镇社区卫生服务中心2022年7月-2023年6月6000张中成药和西药处方的处方点评结果及不合理处方的用药分布情况,以及2023年1月实施改进措施后的改善情况。方法2022年7月-2023年6月每月抽取该院门诊中成药和西药处方500张,共计6000张,分析不合理处方类型分布情况及改进措施实施后的改善情况。结果6000张处方中不合理处方数216张,主要体现在用药不适宜处方(211张),而且在用药不适宜处方中最大占比的处方类型为用法、用量不适宜(168张)。2023年1月实施整改措施后,适应证不适宜、遴选的药品不适宜以及药品剂型或给药途径不适宜的情况均已全部清零,分别由15张、7张以及8张减到0张;用法、用量不适宜处方由103张减少至65张;重复给药处方由11张减少至1张。结论该院在未实施改进措施前,处方开具存在一定的不合理现象,改进措施实施后,改善效果良好,应继续加强行政干预及业务培训等管理措施。Objective To analyze the prescription review results and irrational prescription distribution of 6000 prescriptions of Chinese patent medicine and western medicine in Mapo Town Community Health Service Center of Shunyi District,Beijing from July 2022 to June 2023,and the improvement after the implementation of improvement measures in January 2023.Methods From July 2022 to June 2023,500 prescriptions of Chinese patent medicine and western medicine in the outpatient department of Mapo Town Community Health Service Center in Shunyi District of Beijing were selected monthly,a total of 6000 prescriptions.The distribution of unreasonable prescription types and the improvement after the implementation of improvement measures were analyzed.Results There were 216 unreasonable prescriptions in the 6000 prescriptions,mainly reflected in the inappropriate medication prescriptions(211 prescriptions),and the prescription type with the largest proportion in the inappropriate medication prescriptions was inappropriate usage and dosage(168 prescriptions).After the implementation of rectification measures in January 2023,the indications are not suitable,the selected drugs are not suitable,and the drug dosage form or route of administration are not suitable.All have been cleared,from 15,7 and 8 to 0 respectively;the number of inappropriate usage and dosage was reduced from 103 to 65.Repeated administration was reduced from 11 to 1.Conclusion The unreasonable prescription in our center is serious,and the improvement effect is good after the implementation of the improvement measures.The management measures such as administrative intervention and business training should be strengthened.
