摘　　要：目的探讨血清前白蛋白(PA)、α-羟基丁酸脱氢酶(α-HBDH)对妊娠期肝内胆汁淤积症(ICP)诊断的识别能力及病情程度的判定价值。方法选取在我院诊治的40名ICP患者为研究组,另选取同期在我院进行产检的健康妊娠女性53名为健康对照组,检测血清PA、α-HBDH、TBA、TB、DB水平进行分析比较,绘制受试者工作特征曲线(ROC曲线)并计算曲线下面积(AUC),分析评价PA、α-HBDH的诊断效能。结果研究组PA水平低于对照组,研究组TBA、DB、α-HBDH水平高于对照组(P<0.05);ICP重度组患者的PA水平低于轻度组患者,重度组患者的TBA、DB、α-HBDH水平高于轻度组患者(P<0.05);PA与α-HBDH联合检测诊断妊娠期肝内胆汁淤积症的AUC值高于单项检测,PA与α-HBDH联合检测评估疾病严重程度的AUC值高于单项检测(P<0.05)。结论联合检测血清PA和α-HBDH可以提高诊断妊娠期肝内胆汁淤积症的准确性,对疾病严重程度具有一定的判定价值。Objective To explore the diagnostic recognition capability and the value in assessing the severity of intrahepatic cholestasis of pregnancy(ICP)using serum prealbumin(PA)and alpha-hydroxybutyrate dehydrogenase(α-HBDH).Methods Forty ICP patients treated in our hospital were selected as the study group,and 53 healthy pregnant women undergoing prenatal check-ups during the same period were selected as the healthy control group.Serum levels of PA,α-HBDH,TBA,TB,and DB were measured and analyzed.Receiver operating characteristic(ROC)curves were drawn,and the area under the curve(AUC)was calculated to evaluate the diagnostic efficacy of PA andα-HBDH.Results The PA levels in the study group were lower than those in the control group,while the levels of TBA,DB,andα-HBDH in the study group were higher than those in the control group(P<0.05).The PA levels in the severe ICP group were lower than those in the mild group,while the levels of TBA,DB,andα-HBDH in the severe group were higher than those in the mild group(P<0.05).The combined detection of PA andα-HBDH had a higher AUC value for diagnosing ICP and assessing disease severity than individual tests(P<0.05).Conclusion Combined detection of serum PA andα-HBDH can improve the accuracy of diagnosing intrahepatic cholestasis of pregnancy and has some value in assessing the severity of the disease.
