3D-STI评价甲状腺功能异常患者左心室纵向收缩功能及其与心功能指标及血清心肌损伤标志物的关系研究

摘　　要：目的:探讨三维斑点追踪成像(3D-STI)评价甲状腺功能异常患者左心室纵向收缩功能及其与心功能指标及血清心肌损伤标志物的关系。方法:选择120例甲状腺功能异常患者(研究组)和80例甲状腺功能正常患者(对照组),比较两组整体圆周应变(GCS)、整体纵向应变(GLS)、左心室收缩期整体面积应变(GAS)、左心室射血分数(LVEF)、左心室舒张末容积(LVEDV)、左心室收缩末期内径(LVEDS)和左心室舒张末内径(LVEDD)参数,检测两组血清肌钙蛋白1(cTn1)、肌酸激酶(CK)、CK同工酶(CK-MB)水平。Pearson相关性分析3D-STI参数与常规超声参数及血清cTn1、CK、CK-MB水平的相关性。结果:研究组LVEDD、LVEDS、LVEDV、LVEF与对照组比较差异均无统计学意义(P>0.05),研究组GAS、GLS和GCS绝对值低于对照组,cTn1、CK、CK-MB水平高于对照组(P<0.05)。研究组GAS、GLS和GCS绝对值与血清cTn1、CK、CK-MB水平呈负相关(P<0.05),与LVEDD、LVEDS、LVEDV、LVEF无关(P>0.05)。结论:甲状腺功能异常患者3D-STI参数GAS、GLS和GCS降低,血清cTn1、CK、CK-MB水平升高,3D-STI参数可有效评估甲状腺功能异常患者的左心室纵向收缩功能。Objective:To investigate the evaluation of left ventricular longitudinal systolic function in patients with thyroid dysfunction by three-dimensional speckle tracking imaging(3D-STI)and its relationship with cardiac function indexes and serum myocardial injury markers.Methods:120 patients with abnormal thyroid function(study group)and 80 patients with normal thyroid function(control group)were selected,the parameters of global circumferential strain(GCS),global longitudinal strain(GLS),left ventricular systolic global area strain(GAS),left ventricular ejection fraction(LVEF),left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic diameter(LVEDS)and left ventricular end-diastolic diameter(LVEDD)were compared between two groups,the levels of serum troponin l(cTn1),creatine kinase(CK)and CK isoenzyme(CK-MB)were detected in two groups.The correlation between 3D-STI parameters and conventional ultrasound parameters and the levels of serum cTn1,CK and CK-MB was analyzed by Pearson correlation analysis.Results:There was no significant difference in LVEDD,LVEDS,LVEDV and LVEF between study group and control group(P>0.05),the absolute values of GAS,GLS and GCS in study group were lower than those in control group,The levels of cTn1,CK and CK-MB were higher than those in control group(P<0.05).The absolute values of GAS,GLS and GCS in study group were negatively correlated with the levels of serum cTn1,CK and CK-MB(P<0.05),but not with LVEDD,LVEDS,LVEDV and LVEF(P>0.05).Conclusion:3D-STI parameters GAS,GLS and GCS are decrease in patients with thyroid dysfunction,and the levels of serum cTn1,CK and CK-MB are increase,3D-STI parameters can effectively assess the longitudinal function of the left ventricular contraction in patients with abnormal thyroid function.
