髓核摘除经椎弓根动态固定术与单纯髓核摘除术治疗单节段腰椎间盘突出症的效果对比

摘　　要：目的对比髓核摘除经椎弓根动态固定术与单纯髓核摘除术治疗单节段腰椎间盘突出症(LDH)患者的临床效果。方法回顾性分析我院2020年7月至2023年7月收治的62例单节段LDH患者的临床资料,依据治疗方式的不同分为两组。参照组采用髓核摘除术治疗,研究组采用髓核摘除经椎弓根动态固定术治疗。比较两组的疼痛程度、腰椎功能、并发症及复发率。结果术后1个月,研究组的腰部及腿部VAS评分、ODI评分均明显低于参照组(P<0.05)。两组的术后并发症发生率、复发率比较,差异无统计学意义(P>0.05)。结论与单纯髓核摘除术相比,髓核摘除经椎弓根动态固定术可明显减轻单节段LDH患者的腰部及腿部疼痛感,改善患者的腰椎功能,且未明显增加患者的术后并发症发生风险以及近期复发风险。Objective To compare the clinical effects of nucleus pulposus removal transpedicular dynamic stabilization and simple nucleus pulposus removal in the treatment of patients with single-segment lumbar disc herniation(LDH).Methods The clinical data of 62patients with single-segment LDH admitted to our hospital from July 2020 to July 2023 were retrospectively analyzed.All cases were divided into two groups according to different treatment methods.The reference group was treated with nucleus pulposus removal,and the study group was treated with nucleus pulposus removal transpedicular dynamic stabilization.The pain degree,lumbar function,complications and recurrence rate were compared between the two groups.Results At 1 month after surgery,the VAS scores of waist and leg,and the ODI score in the study group were significantly lower than those in the reference group(P<0.05).No significant difference was found in the incidence of postoperative complications and recurrence rate between the two groups(P>0.05).Conclusions Compared with simple nucleus pulposus removal,nucleus pulposus removal transpedicular dynamic stabilization can significantly reduce the pain degree of waist and leg of patients with single-segment LDH,and improve their lumbar function,without significant increase of occurrence risk of postoperative complications and risk of recent recurrence.
