血清Cys-C、UPCR、血清Cr水平联合检测对糖尿病肾病患者预后的预测价值

摘　　要：目的探讨血清胱抑素C(Cys-C)、尿蛋白与尿肌酐比值(UPCR)、血清肌酐(Cr)联合检测对糖尿病肾病(DN)患者预后的预测价值。方法100例T2DM患者按照m Alb/Cr比值分为单纯DM组、早期DN组和临床DN组,另选取112例健康体检者作对照组,对比四组的血清Cys-C、UPCR、血清Cr水平。对比不同预后DN患者的血清Cys-C、UPCR、血清Cr水平。绘制受试者工作特征(ROC)曲线,分析血清Cys-C、UPCR、血清Cr水平联合检测对DN患者预后的预测价值。结果对照组、单纯DM组、早期DN组、临床DN组的血清Cys-C、UPCR、血清Cr水平依次升高(P<0.05)。预后良好DN患者的血清Cys-C、UPCR、血清Cr水平均低于预后不良DN患者(P<0.05)。ROC曲线显示,血清Cys-C、UPCR、血清Cr水平联合检测预测DN患者预后的AUC值为0.839,高于各指标单独检测(P<0.05)。结论血清Cys-C、UPCR、血清Cr水平与DN患者的预后情况密切相关,上述指标联合检测可作为DN患者预后评估的重要依据。Objective To explore the predictive value of combined detection of serum cystatin C(Cys-C),urinary protein to creatinine ratio(UPCR)and serum creatinine(Cr)for the prognosis of patients with diabetic nephropathy(DN).Methods According to m Alb/Cr ratio,100 patients with T2DM were divided into simple DM group,early DN group and clinical DN group.In addition,112 healthy subjects were selected as the control group.The serum Cys-C,UPCR and serum Cr levels of the four groups were compared.The levels of serum Cys-C,UPCR and serum Cr in DN patients with different prognosis were compared.The receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of combined detection of serum Cys-C,UPCR and serum Cr levels for the prognosis of DN patients.Results The levels of serum Cys-C,UPCR and serum Cr in the control group,simple DM group,early DN group and clinical DN group increased in turn(P<0.05).The levels of serum Cys-C,UPCR and serum Cr in DN patients with good prognosis were lower than those in DN patients with poor prognosis(P<0.05).ROC curve showed that the AUC value of combined detection of serum Cys-C,UPCR and serum Cr levels in predicting the prognosis of DN patients was 0.839,higher than that of single detection of each index(P<0.05).Conclusions The levels of serum Cys-C,UPCR and serum Cr are closely related to the prognosis of DN patients.The combined detection of the above indicators can be used as an important basis for the prognosis evaluation of DN patients.
