彭勋超教授论针刺治疗膝骨关节炎的临证经验

摘　　要：膝骨关节炎是一种常见的慢性骨关节疾病,具有高发病率和致残率。目前西医治疗膝骨关节炎以药物、物理疗法和手术治疗为主,其中药物治疗不良反应多,物理治疗对治疗设备和物理治疗师的要求高,难以推广,手术治疗存在手术风险大、治疗成本高等缺点。针灸在治疗膝骨关节炎方面优势突出、疗效显著。针刺手法繁多,不仅有疗效,而且具有操作方便、价格低廉、安全性高等优点,这对于优化慢性疼痛管理具有重要的指导价值。彭勋超教授认为膝骨关节炎以本痿标痹为核心病机,采用除痹祛邪、通络止痛之法治其标,肝、脾、肾三脏同补治其本,临证强调循经取穴与局部取穴相结合,注重导气针法以加强得气,临床采用传统针刺治疗膝骨关节炎,疗效显著。本研究总结彭勋超教授从本痿标痹论治膝骨关节炎的思路和临床经验,为临床工作者治疗膝骨关节炎提供参考。Knee osteoarthritis(KOA)is a common chronic osteoarthritic disease with high morbidity and disability.Currently,KWestern medicine mainly treat KOA with medicine,physical therapy or surgical treatment.Medicine has many adverse reactions,physical therapy is difficult to be promoted because of the high requirements for treatment equipment and physical therapists,and surgical treatment has the disadvantages of high surgical risk and high treatment cost.Acupuncture and moxibustion show outstanding advantages and remarkable efficacy in the treatment of KOA.There is a wide variety of acupuncture techniques,with clinical effects,and has the advantages of convenient operation,low price,and high safety,which has an important guiding value for optimizing chronic pain management.Professor PENG Xunchao believes that the core pathogenesis of KOA is flaccid basis and paralyzed symptoms,which should be treated by replenishing liver,spleen,and kidneys simultaneously to address the root causes,accompanied by the methods of removing paralysis and eliminating pathogenic factors,and dredging collaterals and relieving pain to relieve symptoms.In clinical practice,it emphasizes the combination of acupoint selection along the meridians and local acupoint selection,focuses on Qi(气)-promoting acupuncture,and uses traditional acupuncture to treat KOA,with remarkable curative effects.In this paper,the thinking and clinical experience of Professor PENG Xunchao in treating KOA is summarized based on the theory of flaccid basis and paralyzed symptoms,providing reference in treating KOA for clinicians.
