摘　　要：类风湿性关节炎是一种慢性、炎性、全身性的自身免疫性疾病,其病理特征是关节炎症、滑膜破坏、血管翳形成,以及骨骼和软骨破坏,通常与持续性的关节炎疼痛、肿胀、僵硬有关,在中医中属于“痹证”“历节”等疾病范畴。目前,类风湿性关节炎的触发因素尚不清楚,遗传、自身免疫和环境因素可能起到致病作用,虽然现代医学取得了显著进展,但尚无能治愈类风湿性关节炎的药物,且仍存在一些不足。中医药在治疗类风湿性关节炎中能够提供更为全面和个性化的治疗,同时减少不良反应的发生,改善患者的整体健康,具有一定优势。角药是以中医基本理论为基础,遵循辨证论治的治疗原则,将三味中药合为一组进行配伍,形成三足鼎立、互为犄角的关系,以起到协同增效作用的药物组合。汪悦教授出生于中医世家,师从其父汪履秋教授,为江苏省名中医、著名风湿病专家,在治疗风湿免疫性疾病方面有丰富的临床经验。笔者对汪悦教授辨治类风湿性关节炎的经验进行整理,对其常用角药的组成、配伍特点、功效及应用等进行归纳,并对临床典型病案进行剖析,以期进一步传承和发扬汪悦教授的学术思想,供临床工作者参考。Rheumatoid arthritis is a chronic,inflammatory,and systemic autoimmune disease,with pathological characteristics of joint inflammation,synovial destruction,pannus formation,and bone and cartilage destruction,is usually associated with persistent arthritis pain,swelling,stiffness,and belongs to the Bi(痹)syndrome,uarthritis and other categories in Chinese medicine.At present,the trigger factors of rheumatoid arthritis are not clear,and genetic,autoimmune and environmental factors may play a pathogenic role in the disease.Although modern medicine has made remarkable progress,there are no drugs that can cure rheumatoid arthritis,and there are still some shortcomings.In the treatment of rheumatoid arthritis,traditional Chinese medicine can provide a more comprehensive and personalized treatment,reducing adverse reactions and improving the overall health of patients,which has certain advantages.The tripartite medicines are based on the basic theory of traditional Chinese medicine and follows the principle of treatment based on syndrome differentiation.It combines three kinds of traditional Chinese medicine as a group to form a relationship of three pillars and each other’s horns,so as to play a synergistic role in clinical application.Professor WANG Yue,born in a family of traditional Chinese medicine,learned from Professor WANG Lvqiu,is a famous TCM practitioner in Jiangsu Province and a famous rheumatologist,and has rich clinical experience in the treatment of rheumatoid immune diseases.The author summarized Professor WANG Yue’s experience in the differentiation and treatment of rheumatoid arthritis,and summarized the composition,compatibility,efficacy and application of commonly used tripartite medicines,in order to further inherit and develop Professor WANG Yue’s academic thought.The typical clinical cases are analyzed,so as to provide reference for clinical workers.
