加味四君子汤辅助常规西药治疗小儿急性阑尾炎术后胃肠功能紊乱临床研究

摘　　要：目的:观察加味四君子汤辅助常规西药治疗小儿急性阑尾炎术后胃肠功能紊乱的临床效果。方法:将86例急性阑尾炎术后胃肠功能紊乱患儿作为研究对象,按随机数字表法分为联合组与参照组各43例。参照组予以常规西药治疗,联合组在常规西药治疗的基础上给予加味四君子汤辅助治疗,共治疗3 d。比较2组临床疗效,比较2组症状缓解时间,并比较2组治疗前后中医证候评分及胃肠激素水平。结果:联合组临床总有效率97.67%,高于参照组83.72%(P<0.05)。治疗后,2组脘腹痞满、食欲不振、疲乏无力、恶心欲吐、大便稀溏等中医证候评分均降低(P<0.05),且联合组低于参照组(P<0.05)。联合组腹痛消失时间、腹胀消失时间、首次排气时间、肠鸣音恢复时间及首次排便时间均短于参照组(P<0.05)。治疗后,2组血浆胃动素(MTL)、血清胃泌素(GAS)水平均升高(P<0.05),且联合组高于参照组(P<0.05)。结论:加味四君子汤辅助常规西药治疗小儿急性阑尾炎术后胃肠功能紊乱疗效显著,可有效促进患儿症状及体征缓解,调节胃肠激素水平。Objective:To observe the clinical effect of Modified Sijunzi Decoction assisting routine western medicine on gastrointestinal dysfunction after surgery for acute appendicitis in children.Methods:A total of 86 cases of children with gastrointestinal dysfunction after surgery for acute appendicitis were divided into the combination group and the reference group according to the random number table method,with 43 cases in each group.The reference group was treated with routine western medicine,and the combination group was additionally treated with Modified Sijunzi Decoction based on routine western medicine.Both groups were treated for three days.Traditional Chinese medicine syndrome scores and gastrointestinal hormone levels before and after treatment were compared between the two groups.The clinical effects and symptom remission time were compared between the two groups.Results:The total clinical effective rate was 97.67%in the combination group,higher than that of 83.72%in the reference group(P<0.05).After treatment,the scores of traditional Chinese medicine syndromes like epigastric fullness,loss of appetite,fatigue and weakness,nausea and vomiting,and loose stool were all decreased in the two groups were decreased(P<0.05),and the scores in the combination group were lower than those in the reference group(P<0.05).The disappearance time of abdominal pain and abdominal distension,the first exhaust time,the bowel sound recovery time and the first defecation time in the combination group were shorter than those in the reference group(P<0.05).After treatment,the levels of plasma motilin(MTL)and serum gastrin(GAS)in the two groups were increased(P<0.05),and the levels in the combination group were higher than those in the reference group(P<0.05).Conclusion:Modified Sijunzi Decoction assisting routine western medicine has a significant curative effect in treating gastrointestinal dysfunction after surgery for acute appendicitis in children,which can effectively promote the relief of symptoms and signs and regulate th
