益经汤联合雷火灸治疗卵巢储备功能下降临床研究

摘　　要：目的:观察益经汤联合雷火灸治疗卵巢储备功能下降(DOR)的临床疗效及对患者卵巢功能、月经的影响。方法:采用随机数字表法将2020年6月—2022年6月收治的60例DOR患者分为观察组、对照组各30例。对照组给予芬吗通联合雷火灸治疗,观察组在对照组基础上加用益经汤治疗。比较2组治疗前后中医证候评分、月经量评分、性激素指标[促黄体生成素(LH)、促卵泡刺激素(FSH)、雌二醇(E_(2))]、卵巢功能指标[卵巢直径、卵巢体积、窦卵泡数量(AFC)、抗苗勒管激素(AMH)],评估2组临床疗效及安全性。结果:观察组总有效率高于对照组(P<0.05)。治疗后,2组中医证候评分及FSH、LH水平均较治疗前降低(P<0.05),E_(2)、卵巢直径、卵巢体积、AFC、AMH水平及月经量评分均升高(P<0.05);观察组治疗后E_(2)、卵巢直径、卵巢体积、AFC、AMH水平及月经量评分高于对照组(P<0.05),中医证候评分及FSH、LH水平低于对照组(P<0.05)。治疗期间,2组无不良反应发生。结论:益经汤联合雷火灸治疗DOR疗效显著,能够改善患者卵巢功能,调节月经,安全性较高。Objective:To observe the clinical effect of Yijing Decoction combined with thunder-fire moxibustion on diminished ovarian reserve(DOR)and its effects on ovarian function and menstruation.Methods:A total of 60 cases of DOR patients admitted from June 2020 to June 2022 were divided into the observation group and the control group according to the random number table method,with 30 cases in each group.The control group was additionally treated with Femoston and thunder-fire moxibustion,and the observation group was additionally treated with Yijing Decoction.Traditional Chinese medicine(TCM)syndrome scores,menstrual volume scores,sex hormone indicators[luteinizing hormone(LH),folliclestimulating hormone(FSH),estradiol(E_(2))],and ovarian function indicators[ovarian diameter,ovarian volume,antral follicle count(AFC),and anti-Mullerian hormone(AMH)]were compared between the two groups before and after treatment.Clinical effects and safety were evaluated in the two groups.Results:The total effective rate in the observation group was higher than that in the control group(P<0.05).After treatment,TCM syndrome scores and the levels of FSH and LH in the two groups were decreased when compared with those before treatment(P<0.05),and the menstrual volume scores and the levels of E_(2),ovarian diameter,ovarian volume,AFC,and AMH were increased when compared with those before treatment(P<0.05).After treatment,the menstrual volume score and the levels of E_(2),ovarian diameter,ovarian volume,AFC,and AMH in the observation group were higher than those in the control group(P<0.05),and TCM syndrome scores and the levels of FSH and LH in the observation group were lower than those in the control group(P<0.05).During treatment,there was no adverse reactions occurred in the two groups.Conclusion:Yijing Decoction combined with thunder-fire moxibustion has significant curative effects in the treatment of DOR,which can improve ovarian function and regulate menstruation with high safety.
