摘 要:目的:观察消渴方加减联合短期胰岛素强化治疗2型糖尿病初发患者的疗效。方法:选取100例2型糖尿病初发患者,根据治疗方式不同分为对照组、观察组各50例。对照组应用短期胰岛素强化治疗,观察组在对照组基础上加用消渴方加减治疗。比较2组临床疗效及不良反应发生率,比较2组治疗前后中医证候积分、空腹血糖(FBG),胰岛β细胞功能指数(HOMA-β)、血糖波动参数[日平均血糖波动幅度(MAGE)、全天血糖标准差(SDBG)]的变化。结果:观察组有效率为96.00%,对照组为78.00%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组中医证候积分均较治疗前下降(P<0.05),观察组中医证候积分低于对照组(P<0.05)。治疗后,2组FBG水平均较治疗前下降(P<0.05),HOMA-β均较治疗前上升(P<0.05);观察组FBG水平低于对照组(P<0.05),HOMA-β高于对照组(P<0.05)。治疗后,2组血糖波动参数MAGE、SDBG均较治疗前下降(P<0.05),观察组MAGE、SDBG均低于对照组(P<0.05)。观察组不良反应发生率为2.00%,对照组为10.00%,2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:消渴方加减联合短期胰岛素强化治疗2型糖尿病效果较好,能改善症状,降低血糖水平,改善胰岛功能,治疗稳定性强,安全性高。Objective:To observe the curative effect of modified Xiaoke Prescription combined with intensive therapy with short-term insulin pump for incipience patients with type 2 diabetes mellitus.Methods:A total of 100 cases of incipience patients with type 2 diabetes mellitus were selected and divided into the control group and the observation group according to different treatment,with 50 cases in each group.The control group received intensive therapy with short-term insulin pump,and the observation group received additional treatment with Xiaoke Prescription based on the treatment of the control group.The clinical effects and incidence of adverse reactions were compared between the two groups.The changes in traditional Chinese medicine(TCM)syndrome scores,fasting blood glucose(FBG),homeostasis model assessment ofβ-cell function(HOMA-β),and blood glucose fluctuation parameters[mean amplitude of glycemic excursions(MAGE)and standard deviation of mean blood glucose(SDBG)]were compared before and after treatment between the two groups.Results:The effective rate was 96.00%in the observation group,and 78.00%in the control group,the difference being significant(P<0.05).After treatment,the TCM syndrome scores in the two groups were decreased when compared withthose before treatment(P<0.05),and the TCM syndrome scores in the observation group were lower thanthose in the control group(P<0.05).After treatment,the FBG levels in the two groups were decreasedwhen compared with those before treatment(P<0.05),HOMA-βwere increased when compared withthose before treatment(P<0.05);the FBG level in the observation group was lower than that in the controlgroup(P<0.05),HOMA-βwere higher than that in the control group(P<0.05).After treatment,theblood glucose fluctuation parameters including MAGE and SDBG in the two groups were decreased whencompared with those before treatment(P<0.05),and the MAGE and SDBG in the observation group werelower than those in the control group(P<0.05).The incidence of adverse reactions was 2.00%in theobserv
