摘 要:目的:探究高频超声在全层撕裂型肩袖损伤诊断中的应用价值。方法:选取2021年1月—2023年1月滨州市中心医院收治的80例肩袖损伤患者作为研究对象,根据肩关节镜检查结果分为部分撕裂组和全层撕裂组,每组40例。两组患者均接受高频超声检查,比较两组超声特征(腱鞘积液、肩袖钙化、肌腱变性、肌腱增厚),肌筋膜厚度、冈下肌腱厚度、冈上肌腱厚度。结果:高频超声诊断全层撕裂型肩袖损伤36例,部分撕裂型肩袖损伤39例。全层撕裂组腱鞘积液、肩袖钙化、肌腱变性、肌腱增厚占比高于部分撕裂组(P<0.001)。全层撕裂组肌筋膜、冈下肌腱和冈上肌腱均比部分撕裂组更厚(P<0.001)。结论:不同撕裂程度肩袖损伤患者的超声检查特征及肌筋膜、冈下肌腱和冈上肌腱厚度存在差异,超声检查可作为临床诊断肩袖损伤的辅助检查方法。Objective:To explore the application value of high-frequency ultrasound in diagnosis of full-thickness tear rotator cuff injury.Methods:A total of 80 patients with rotator cuff injury who were admitted to Binzhou Central Hospital from January 2021 to January 2023 were selected as the study subjects.According to the results of shoulder arthroscopy,the patients were divided into partial tear group and full-thickness tear group,with 40 patients in each group.All patients in two groups underwent high-frequency ultrasound.The ultrasound characteristics(tendon sheath effusion,rotator cuff calcification,tendon degeneration,tendon thickening),myofascial thickness,infraspinatus tendon thickness,and supraspinatus tendon thickness were compared between the two groups.Results:High-frequency ultrasound diagnosis results showed 36 cases with full-thickness tear rotator cuff injury and 39 cases with partial tear rotator cuff injury.The proportions of tendon sheath effusion,rotator cuff calcification,tendon degeneration and tendon thickening in the full-thickness tear group were higher than those in the partial tear group(P<0.001).The myofascia,infraspinatus tendon and supraspinatus tendon in the full-thickness tear group were thicker than those in the partial tear group(P<0.001).Conclusion:There are differences in ultrasonography characteristics and myofascial thickness,infraspinatus tendon thickness,and supraspinatus tendon thickness in patients with different tear severities,and ultrasound can be used as an auxiliary examination method for clinical diagnosis of rotator cuff injury.
