摘  要:目的:探讨线性探针杂交(HAIN)技术在结核病诊断及耐药检测中的应用价值。方法:选取2015年1月—2021年12月于江苏大学附属镇江三院就诊的疑似结核病患者345例进行研究,收集患者痰标本进行HAIN检测和罗氏固体培养,并对罗氏固体培养为阳性的标本进行传统药敏试验和HAIN耐药检测。分析HAIN技术诊断结核病、耐药性的效能及对耐异烟肼(INH)和耐利福平(RFP)、多重耐药性(MDR)的检出率。结果:345例疑似结核病患者中,罗氏固体培养检测诊断阳性181例,HAIN技术检测诊断阳性180例。HAIN检测与罗氏固体培养检测结果的一致性较高(Kappa=0.861,P<0.001)。传统药敏试验检出耐药92例,HAIN检测检出耐药91例。HAIN技术与传统药敏试验结果的一致性较高(Kappa=0.884,P<0.001)。HAIN检测耐INH、耐RFP、MDR的检出率与传统药敏试验对比,差异无统计学意义(P>0.05)。结论:HAIN技术在结核病诊断及耐药检测中的应用价值较高,可用于结核病早期筛查和诊断。Objective:To explore the application value of linear probe hybridization(HAIN)technique in tuberculosis diagnosis and drug resistance detection.Methods:A total of 345 suspected tuberculosis patients admitted to Zhenjiang Third Hospital Affiliated to Jiangsu University from January 2015 to December 2021 were selected for the study.Sputum specimens of the patients were collected for HAIN detection and Roche solid culture.The traditional drug sensitivity test and HAIN resistance test were carried out on the specimens that were positive by Roche solid culture.The efficacy of HAIN technique in the tuberculosis diagnosis and drug resistance,and the detection rates of isoniazid(INH)resistance,rifampicin(RFP)resistance and multidrug resistance(MDR)were analyzed.Results:Among the 345 suspected tuberculosis patients,181 cases were positive by Roche solid culture test and 180 cases were positive by HAIN technique test.The results of HAIN test and Roche solid culture test were highly consistent(Kappa=0.861,P<0.001).Drug resistance was detected in 92 cases by traditional drug sensitivity test and in 91 cases by HAIN test.The results of HAIN test and traditional drug sensitivity test were highly consistent(Kappa=0.884,P<0.001).There was no significant difference in the detection rates of INH resistance,RFP resistance and MDR between HAIN test and traditional drug sensitivity test(P>0.05).The coincidence rates of INH resistance,RFP resistance and MDR by HAIN test and traditional drug sensitivity test were 95.00%,96.43%and 95.83%,respectively.Conclusion:HAIN technique has high application value in tuberculosis diagnosis and drug resistance detection,and can be used for early screening and diagnosis of tuberculosis.
