摘　　要：目的通过荟萃分析系统评价体外膜肺氧合技术(Extracorporeal Membrane Oxygenation,ECMO)对暴发性心肌炎(Fulminant myocarditis,FM)患者的预后及所致并发症的影响。方法计算机检索PubMed、Cochrane Library、Web of Science、中国生物医学文献数据库、万方数据库、维普数据库等,并且追溯了相关文献的参考文献以及相关联文献,检索时间从建库起至2023年11月30日。由2名研究者按照纳入和排除标准独立筛选文献、提供相关资料并评价文献质量后,对相关证据进行梳理与分析,使用Stata 16.0和Stata 14.0软件进行Meta分析。结果共纳入15篇文献,共525例患者。Meta分析显示应用ECMO治疗的暴发性心肌炎患者的死亡率为31%(95%CI:0.27~0.36),出现如出血、多器官功能衰竭、肢端缺血等并发症。结论暴发性心肌炎患者的治疗过程中,应用ECMO支持治疗的患者死亡率为31%,且在治疗过程中伴随并发症的出现。Objective To evaluate the effect of Extracorporeal Membrane Oxygenation(ECMO)on the prognosis and the complications of patients with Fulminant myocarditis(FM)by means of a meta-analysis.Methods PubMed,Cochrane Library,Web of Science,China Biomedical Literature Database,Wanfang Database and VIP database were searched by computer,and the references and related literatures were traced back from the establishment of ECMO database to November 30,2023.According to the inclusion and exclusion criteria,two researchers independently screened the literature,provided relevant data and evaluated the literature quality.Then,the relevant evidence was combed and analyzed.Stata 16.0 and Stata 14.0 were used for meta-analysis.Results A total of 15 literatures were included,involving 525 patients.Meta-analysis showed that the mortality rate in patients with fulminant myocarditis treated with ECMO was 31%(95%CI:0.27~0.36).However,some complications such as bleeding,multiple organ failure,and limb ischemia were also caused.Conclusion In the course of treatment of fulminant myocarditis,the mortality rate of patients receiving ECMO support treatment is 31%,and some complications may occur during the treatment.
