加强布加综合征介入治疗后再狭窄研究

摘　　要：介入治疗作为布加综合征首选临床治疗方法已在国内外得到广泛认可。经查询万方医学数据库,1990年以来我国布加综合征介入治疗文献超过110余篇,总病例数超过12600例,国内布加综合征介入治疗技术水平已达到国际领先水平。随着介入治疗布加综合征病例数增加,患者生存时间延长和对生存质量意识提高,临床发现布加综合征介入治疗后再狭窄发生率已超过21%。布加综合征介入治疗后再狭窄可表现为急性再狭窄、慢性再狭窄、反复再狭窄和支架植入引起的医源性再狭窄,再狭窄发生机理尚不清楚。血管弹性回缩、球囊扩张导致局部组织损伤与修复、病因持续存在、隔膜再形成、支架刺激局部导致组织增生以及术者技术水平差异对再狭窄发生负有一定责任。加强布加综合征介入术后再狭窄的基础、器材、药物及预测生信模型建立等研究,是目前临床面临的重要课题。Interventional therapy has been widely recognized as the preferred clinical treatment for Budd-Chiari syndrome at home and abroad.Through searching the literature concerning the interventional therapy for Budd-Chiari syndrome patients in China from Wanfang medical database,more than 110 academic papers with over 12600 cases since 1990 in China have been obtained.The technical level of domestic interventional therapy for Budd-Chiari syndrome has already reached the international leading level.With the increasing of the number of Budd-Chiari syndrome cases receiving interventional therapy,the survival time of patients has been extended,the patient′s awareness of life quality has been improved,and the incidence of the restenosis of Budd-Chiari syndrome after interventional therapy is more than 21%.Clinically,the restenosis of Budd-Chiari syndrome after interventional treatment can be manifested as acute restenosis,chronic restenosis,repeated restenosis and iatrogenic restenosis caused by stent implantation.The mechanism of restenosis is still unclear.The vascular elastic retraction,the local tissue injury and repair caused by balloon dilation manipulation,the persistence of the etiology,the re-formation of membrane,the local tissue proliferation induced by stent stimulation,and the technical skill level of the interventional operators,etc.are all related to the occurrence of restenosis.Therefore,it is an important clinical topic to strengthen the research on the basic etiology,equipment,drugs and the establishment of predictive biological model of the restenosis of Budd-Chiari syndrome after interventional therapy.
