摘 要:重症监护室(ICU)病人大多存在高营养风险,高营养风险是增加患者住院率、病死率和并发症的独立危险因素,给患者家庭及社会带来极大经济负担。目前,临床上营养治疗包括肠内营养及肠外营养,优选肠内营养,但仍疗效欠佳。究其原因,是胃肠功能障碍导致肠内营养受阻。中医药基于“脾胃运化”理论调整脾胃功能,进而促进肠内营养的启动及达标,对ICU高营养风险患者的营养治疗具有积极作用。本文结合“脾胃运化”理论及现代医学认识,为ICU患者高营养风险的防治提供新的临床治疗方法及研究思路。Most patients in the intensive care unit(ICU)have high nutritional risk,which is an independent risk factor for increasing the hospitalization rate,mortality,and incidence of complications and brings a heavy economic burden to patients'families and the society.At present,clinical nutritional therapies include enteral nutrition and parenteral nutrition.Although enteral nutrition is preferred,it has poor effects due to gastrointestinal dysfunction.Traditional Chinese medicine(TCM)adjusts spleen and stomach functions based on the theory of spleen and stomach governing transportation and transformation to promote the initiation and qualification of enteral nutrition,demonstrating a positive effect on the nutritional treatment of patients with high nutritional risk in the ICU.The combination of the TCM theory of spleen and stomach governing transportation and transformation and modern medical understanding provides a new clinical treatment method and research idea for the prevention and treatment of high nutritional risk in ICU patients.
