中医方法治疗严重颈椎管狭窄脊髓受压2例

摘　　要：本文报道2例严重颈椎管狭窄脊髓受压的病案,患者有病情加重导致瘫痪等严重后果的风险,有手术指征,但拒绝手术治疗,要求保守治疗。2例患者均合并腰椎间盘突出症,其中1例患者合并双膝骨性关节炎,病情重,病情复杂,治疗难度大。笔者秉承中国民间“一针二灸三用药”理念,以中医整体观念为指导,以颈椎管狭窄致脊髓受压为关键着手点,以病变颈椎节段为治疗重点,采用针灸、小针刀、穴位注射、中药汤剂口服等有确切疗效的中医方法,取得了很好的治疗效果,让患者免于手术治疗。This paper presented two cases of severe cervical spinal stenosis with spinal cord compression,where the patients were at risk of serious consequences,such as paralysis,due to disease progression.Both patients had indications for surgery but refused surgical treatment and requested conservative management.Additionally,both patients also had lumbar disc herniation,and one of them had severe knee osteoarthritis,making the condition complex and difficult to treat.Adhering to the traditional Chinese folk concept of"first acupuncture,second moxibustion,and third medications",the author utilized a holistic approach rooted in traditional Chinese medicine(TCM).The treatment focused on the compression of the spinal cord caused by cervical spinal stenosis,with particular attention to the affected cervical spine segment.A combination of acupuncture,small needle knife therapy,acupoint injection,and oral administration of TCM decoctions was employed.These methods,with established efficacy,yielded positive results and successfully spared the patients from surgery.
