摘　　要：慢性咳嗽是呼吸系统常见的临床症状之一,病因复杂,以上气道咳嗽综合征(UACS)、咳嗽变异性哮喘(CAV)、变应性咳嗽(AC)、胃食道返流性咳嗽(GERC)、嗜酸粒细胞性支气管炎(EB)等最为常见,常迁延难愈,给患者的生活和工作带来一定的影响。《诸病源候论》首先记述了久咳嗽的中医病候,病因多为外感咳嗽久治不愈、嗜食肥甘厚味、饮酒过度、劳累过度等,病机上重视风邪犯肺、邪热结咽、胃气上逆、肝火犯肺、诸脏先伤后传于肺和外感内伤互为因果导致气机逆乱,肺失宣降。其病位主要在肺,与肝、脾、肾、心、胃等密切相关。治疗上一方面要注重调畅气机,另一方面可综合运用经典方合方与岭南特色中草药结合,再结合现代医学对于慢性咳嗽的认识,通过辨病与辨证相结合的方式治疗可获良好疗效。Chronic cough is one of the common clinical symptoms of the respiratory system,with a complex etiology.The most common types include upper airway cough syndrome(UACS),cough variant asthma(CAV),atopic cough(AC),gastroesophageal reflux cough(GERC),and eosinophilic bronchitis(EB).These conditions often persist and are difficult to treat,significantly impacting patients'daily lives and work.The Treatise on the Sources and Stages of Various Diseases(Zhu Bing Yuan Hou Lun)first records the traditional Chinese medicine(TCM)syndromes of chronic cough.The causes are often external pathogens causing persistent cough,excessive consumption of fatty and rich foods,excessive alcohol intake,and overwork.In terms of pathogenesis,attention should be paid to factors such as wind pathogens invading the lungs,pathogenic heat blocking the throat,stomach qi inversion,liver fire attacking the lungs,and the injury of other organs that is later transmitted to the lungs.External and internal injuries are interrelated,leading to a disturbance of qi flow and the failure of the lungs to regulate and descend.The primary location of the disease is the lungs,but it is closely related to the liver,spleen,kidney,heart,and stomach.In terms of treatment,it is important to focus on regulating the flow of qi.Additionally,a combination of classic TCM formulas and Lingnan characteristic herbs should be used.When combined with modern medical understanding of chronic cough,a treatment approach integrating both disease and syndrome differentiation can achieve good therapeutic effects.
