摘　　要：目的探讨改良缝合技术在经阴道分娩会阴裂伤的初产妇中的应用效果。方法选取2022年10月、12月在南京大学医学院附属鼓楼医院产科门诊建卡、常规产检、足月经阴道自然分娩Ⅰ~Ⅱ度会阴裂伤的初产妇146例为研究对象,80例采用传统缝合技术的为对照组,66例采用改良缝合技术的为观察组。观察两组产妇产后会阴疼痛、水肿发生率、伤口愈合度、产后会阴舒适度、尿潴留发生率、下床活动时间<6 h、性生活恢复时间及新生儿的早期吮吸率等。结果与对照组相比,观察组的产妇产后会阴疼痛、水肿发生率、伤口愈合度、产后会阴舒适度、尿潴留发生率、下床活动时间<6 h占比、性生活恢复时间均优于对照组(P<0.05),而两组新生儿的早期吮吸率无统计学差异(P>0.05)。结论会阴改良缝合技术能够有效减轻初产妇疼痛,明显提升初产妇的舒适度,值得临床推广。Objective To explore the application effect of improved suture technique on primipara with perineal laceration in vaginal delivery.Methods From October to December 2022,146 primiparas withⅠ~Ⅱdegree perineal laceration who underwent card establishment,routine prenatal examination and natural vaginal delivery of full term in the Obstetrics Clinic of Drum Tower Hospital,Nanjing University School of Medicine were selected as the research objects,80 cases were treated with traditional suture technology as the control group,and 66 cases with modified suture technology were selected as the observation group.The incidence of postpartum perineal pain,edema,wound healing,postpartum perineal comfort,urinary retention,time of getting out of bed<6 h,recovery time of sexual life and early sucking rate of newborns were observed in the two groups.Results Compared with the control group,the rates of postpartum perineal pain,edema,wound healing,postpartum perineal comfort,urinary retention,time to get out of bed(<6 h),and time to recover sexual life were better in the observation group(P<0.05),and there was no significant difference in the rate of newborn's early sucking between the two groups(P>0.05).Conclusion The modified suture technique of perineum can effectively reduce the pain of primiparous women and significantly improve the comfort of primiparous women,which is worthy of clinical promotion.
