益气化瘀方联合腕踝针配合曼月乐治疗气虚血瘀型子宫内膜异位症的临床观察

摘　　要：目的观察益气化瘀方联合腕踝针配合曼月乐治疗气虚血瘀型子宫内膜异位症(EMS)的临床效果。方法选取2023年1月至9月期间秦皇岛市妇幼保健院收治的气虚血瘀型EMS患者80例,按随机数字表法分为对照组(40例,曼月乐治疗)和研究组(40例,腕踝针联合益气化瘀方口服配合曼月乐治疗),两组均治疗3个月。比较两组治疗前后视觉模拟评分法(VAS)评分、痛经症状积分、糖类抗原125(CA125)、血管内皮生长因子(VEGF)、子宫体积、子宫内膜厚度以及治疗效果和阴道不规则出血情况。结果与治疗前比较,治疗3个月后两组VAS评分、痛经症状积分、CA125、VEGF、子宫体积、子宫内膜厚度均降低(P<0.05),且研究组较对照组下降幅度更大,差异有统计学意义(P<0.05)。研究组的治疗总有效率(97.50%)较对照组(82.50%)更高、阴道不规则出血率更低(5.00%vs.20.00%),差异有统计学意义(P<0.05)。结论益气化瘀方联合腕踝针配合曼月乐治疗气虚血瘀型EMS患者,可减轻疼痛感和痛经症状,降低血清CA125、VEGF,缩小子宫体积、子宫内膜厚度,提高治疗效果,减少阴道不规则出血。Objective To observe the clinical effect of Yiqi Huayu recipe,wrist-ankle acupuncture combined with Mirena in treating endometriosis(EMS)with qi deficiency and blood stasis.Methods 80 EMS patients with qi deficiency and blood stasis from January to September 2023 in Qinhuangdao Maternal and Child Health Hospital were divided into control group(40 cases,treated with Mirena)and study group(40 cases,treated with Yiqi Huayu recipe,wrist-ankle acupuncture combined with Mirena)according to the random number table method.Both groups were treated for 3 months.The visual analogue score(VAS),dysmenorrhea symptom score,carbohydrate antigen 125(CA125),vascular endothelial growth factor(VEGF),uterine volume,endometrial thickness,treatment effect and irregular vaginal bleeding before and after treatment were compared between the two groups.Results Compared with those before treatment,VAS score,dysmenorrhea symptom score,CA125,VEGF,uterine volume and endometrial thickness were all decreased in the two groups after 3 months of treatment(P<0.05),and the decrease in the study group was greater than that in the control group,and the difference was statistically significant(P<0.05).The total effective rate of treatment in the study group(97.50%)was higher than that in the control group(82.50%),and the rate of irregular vaginal bleeding was lower(5.00%vs.20.00%),and the difference was statistically significant(P<0.05).Conclusion Yiqi Huayu recipe,wrist-ankle acupuncture combined with Mirena can relieve pain and dysmenorrhea symptoms,reduce serum CA125,VEGF,uterine volume and endometrial thickness,improve therapeutic effect and reduce irregular vaginal bleeding.
