摘　　要：目的:分析补偿制度改革下公立医院的运营情况,提出优化建议。方法:梳理公立医院补偿制度,描述性分析2015-2023年公立医院运营总量和结构变化,通过访谈关键知情人了解原因。结果:2015-2023年,公立医院服务量和技术难度增加,但服务宏观结构需要调整、优化,住院服务增速高;补偿结构优化,但医疗收入结构未达预期目标。结论:公立医院补偿政策尚有完善空间,政府应宏观把控资源配置,发挥财政和医保的引导作用;公立医院应加强内部管理,着力提升运营效益。Objective To analyze the operation of public hospitals under the reform of compensation related systems, and to propose policy recommendations for optimizing the operation of public hospitals from both the government and hospital perspectives. Methods Literature review was conducted to sort out relevant compensation systems, the total and structural changes in the operation of public hospitals from 2015 to 2023 were analyzed, and key informants were interviewed to understand the reasons and countermeasures. Results From 2015to 2023, the service volume and technical difficulty of public hospitals increased, but the macro structure of services needs to be adjusted,and the growth rate of inpatient services is high. The compensation structure was optimized, but the structure of medical revenue did not meet the expected target. Conclusion There is still room for improvement in the compensation policy of public hospitals. The government should control the allocation of resources in a macro way and play the guiding role of finance and medical insurance. Public hospitals should strengthen internal management and strive to improve operation efficiency.
