加减黄连温胆汤治疗痰湿壅盛型代谢综合征的临床疗效观察

摘　　要：目的:观察加减黄连温胆汤治疗痰湿壅盛型代谢综合征患者的临床疗效。方法:选取2021年9月-2022年9月于黑龙江中医药大学附属第一医院心内科就诊的痰湿壅盛型代谢综合征(MS)患者68例,采用随机数字表法将其随机分为对照组和治疗组,每组34例,对照组给予常规西药治疗,并辅以健康教育,治疗组在对照组治疗方案基础上,加用加减黄连温胆汤,两组治疗周期均为6周。比较两组患者治疗前后中医证候疗效、血压(舒张压、收缩压)、腹围、血糖指标[胰岛素抵抗指数(HOMA-IR)、空腹胰岛素(FINS)、空腹血糖(FPG)]、体质量指数(BMI)、血脂指标[高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、甘油三酯(TG)]。结果:治疗6周后,两组患者腹围、舒张压、收缩压、HOMA-IR、FINS、FPG、LDL-C、TC、TG、BMI均明显降低(P<0.05),同时治疗组显著低于对照组(P<0.05);两组患者HDL-C较治疗前明显增高(P<0.05),同时治疗组显著高于对照组(P<0.05)。治疗组和对照组的中医证候疗效总有效率分别是94.1%(32/34)和79.4%(27/34),治疗组总有效率显著高于对照组(P<0.05)。结论:加减黄连温胆汤可有效改善痰湿壅盛型MS的症状,并降低患者腹围、血压、BMI、血脂指标、血糖指标。Objective:To observe the clinical efficacy of modified Huanglian Wendan Decoction in treating patients with phlegm-dampness Congestion syndrome of metabolic syndrome(MS).Methods:A total of 68 patients diagnosed with phlegm-dampness Congestion type MS who visited the Department of Cardiology at The First Affiliated Hospital of Heilongjiang University of Chinese Medicine from September 2021 to September 2022 were selected.Patients were randomly divided into two groups:the control group and the treatment group,with 34 patients in each group.The control group received conventional Western medicine treatment along with health education,while the treatment group received the same treatment as the control group,plus modified Huanglian Wendan Decoction.Both groups underwent a 6-week treatment period.The efficacy of TCM syndromes,as well as various indicators including blood pressure(diastolic and systolic),waist circumference,blood glucose[homeostasis model assessment of insulin resistance(HOMA-IR),fasting insulin(FINS),fasting blood glucose(FPG)],body mass index(BMI),and blood lipid levels[high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),total cholesterol(TC),triglycerides(TG)]were compared before and after treatment.Results:After 6 weeks of treatment,waist circumference,diastolic and systolic blood pressure,HOMA-IR,FINS,FPG,LDL-C,TC,TG,and BMI were significantly lower in both groups(P<0.05),with the treatment group showing significantly better results than the control group(P<0.05).HDL-C levels were significantly higher than before treatment in both groups(P<0.05),and the treatment group had significantly higher HDL-C compared to the control group(P<0.05).The total effective rate of TCM syndrome treatment in the treatment group was 94.1%(32/34),significantly higher than 79.4%(27/34)in the control group(P<0.05).Conclusion:Modified Huanglian Wendan Decoction is effective in improving the symptoms of phlegm-dampness Congestion type MS,and it can reduce waist circumference,blood pressu
