从叶天士虚劳病“经-络-奇”辨治虚性体质艾滋病探微

摘　　要：虚性体质人群在艾滋病各病程阶段均有较大占比,并较实性体质有着更低下的免疫水平,其中尤以气虚质、阳虚质最为突出,这可能对患者的免疫重建、疾病的进展及预后产生不良影响,故尽早采取有效干预措施具有必要性。叶天士虚劳病在病机表现、病位及病证阶段上均与其有相通之处,在虚劳病中叶天士强调“初病气结在经,久病血伤入络”“由脏腑络伤,已及奇经”的先后病证阶段,文章以叶天士所提出的虚劳独特的“经-络-奇”病证阶段为切入点,提出艾滋病无症状期气虚质相当于虚劳经络阶段,而中后期出现的阳虚质实为虚劳奇络阶段,并根据各阶段不同的病机特点,提出“甘温养元,流行通络”“通补奇络,缓攻宿敌”等辨治方法,以期为虚性体质艾滋病的个体化诊疗提供理论参考。People with deficiency constitution have a larger proportion in all stages of AIDS and have lower immunity level than those with solid constitution,especially Qi deficiency and Yang deficiency,which may adversely affect the immune reconstruction,disease progression and prognosis of the patients,so it is necessary to take effective interventions as early as possible.Ye Tianshi's deficiency labor disease in the disease mechanism performance,disease location and disease stage have similarities with it,in the deficiency labor disease Ye Tianshi emphasized that"the initial disease gas knot in the meridian,prolonged disease blood injuries into the collaterals","by the visceral organs and collaterals injuries,and has been extra meridian"of the successive stages of the disease,so this paper tries to learn from its theory of"deficiency labor".Therefore,this paper takes the unique"meridian-conduit-conduit"stage of deficiency labor proposed by Ye Tianshi as the starting point,proposes that the Qi deficiency in the asymptomatic period of AIDS is equivalent to the meridian-conduit stage of deficiency labor,and the Yang deficiency in the middle and late stages is the Qi-conduit stage of deficiency labor.Based on the different characteristics of the disease mechanism in each stage,we proposed"sweet warmth to nourish the vital energy,popularize the circulation of the channels"and"tonifying the channels and slowing down the attack on the enemy"as the methods of treatment.and the identification and treatment ideas of AIDS patients with deficiency constitution,with a view to providing theoretical references for the individualized diagnosis and treatment of AIDS with deficiency constitution.
