摘 要:目的观察清胰Ⅱ号制剂对胡蜂毒致多脏器功能障碍综合征(MODS)大鼠多脏器功能的保护作用。方法将SPF级健康雄性SD大鼠36只随机数字表法分为模型组、空白对照组及清胰Ⅱ号组,每组12只。清胰Ⅱ号组和模型组利用静脉注射胡蜂毒的方法建立MODS大鼠模型,空白对照组静脉注射等体积0.9%氯化钠注射液,清胰Ⅱ号组同时鼻饲清胰Ⅱ号制剂10 ml/kg,而模型组和对照组鼻饲等体积0.9%氯化钠注射液。干预24 h、72 h后,比较3组心肝肾功能指标[血清肌钙蛋白T(cTnT)、丙氨酸氨基转移酶(ALT)、血肌酐(SCr)]、血清肠道功能指标[二胺氧化酶(DAO)、内毒素(ETX)]、血清炎性因子[白介素(IL)-6、IL-8、IL-1β、肿瘤坏死因子-α(TNF-α)]水平及结直肠组织中p-Akt/Akt、p65、p-p65的表达。结果(1)心、肝、肾、肠道功能指标及炎性因子:与空白对照组比较,模型组大鼠干预24 h、72 h的血清cTnT、ALT、SCr、DAO、ETX、IL-6、IL-8、IL-1β、TNF-α水平均升高(P<0.05);与模型组比较,清胰Ⅱ号组血清干预24 h、72 h的cTnT、ALT、SCr、DAO、ETX、IL-6、IL-8、IL-1β、TNF-α水平均降低(P<0.05)。(2)结直肠组织蛋白表达:与空白对照组比较,模型组结肠及直肠组织中p-Akt/Akt、p65、p-p65均上调,但清胰Ⅱ号组结肠及直肠组织中p-Akt/Akt、p65、p-p65低于模型组(P<0.05)。结论清胰Ⅱ号制剂可降低血清cTnT、ALT、SCr、DAO水平,通过介导Akt/NF-κB信号通路对胡蜂毒致MODS大鼠的心肝肾及肠道等多器官具有良好的修复和保护作用。Objective To observe the protective effect of QingyiⅡpreparation on multiple organ functions in rats with MODS caused by wasp venom.Methods Thirty-six healthy male SD rats of SPF grade were randomly divided into model group,blank control group,QingyiⅡgroup,12 rats in each group.The model group and QingyiⅡgroup were injected with wasp venom intravenously to establish the MODS rat model,the blank control group was injected with an equal volume of saline intravenously,and the QingyiⅡgroup was given 10 ml/kg of the QingyiⅡpreparation intranasally,whereas the model group and the blank control group were given an equal volume of saline by gavage via nasogastric gavage.After 24 hours and 72 hours of intervention,cardiac,hepatic and renal function indexes(cTnT,ALT,SCr),serum intestinal function indexes(DAO,ETX),serum inflammatory factors(IL-6,IL-8,IL-1β,TNF-α)levels and the expression of p-Akt/Akt,p65,p-p65 in colorectal tissues of the three groups were compared.Results(1)Cardiac,hepatic,renal and intestinal function indexes and inflammatory factors:compared with the blank control group,the levels of cTnT,ALT,SCr,DAO,ETX,IL-6,IL-8,IL-1β,TNF-αin the rats of the model group were elevated at 24 hours and 72 hours of intervention(P<0.05).Compared with the model group,the levels of cTnT,ALT,SCr,DAO,ETX,IL-6,IL-8,IL-1βand TNF-αof the were reduced in QingyiⅡgroup at 24 hours and 72 hours of intervention(P<0.05).(2)Protein expression in colon and rectal tissues:compared with the blank control group,p-Akt/Akt,p65,p-p65 were up-regulated in colon and rectal tissues of the model group,but the p-Akt/Akt,p65,p-p65 in the colon and rectum tissues of the QingyiⅡgroup was lower than that of the model group(P<0.05).Conclusion QingyiⅡpreparation can reduce the levels of serum cTnT,ALT,SCr,DAO,and has good repair and protective effects on multiple organs such as heart,liver,kidney and intestines in rats with wasp induced MODS by mediating the Akt/NF-κB signaling pathway.
