单侧经皮椎体成形术治疗胸腰椎压缩性骨折的效果

摘　　要：目的分析单侧经皮椎体成形术(PVP)治疗胸腰椎压缩性骨折的效果。方法选取2021年6月至2023年6月延边朝医医院脊柱外科收治的86例胸腰椎压缩性骨折患者,随机数字表法将患者划分成两组,每组43例。两组患者均接受PVP治疗,观察组采取单侧穿刺,对照组采取双侧穿刺。比较两组的手术指标、Oswestry功能障碍指数(ODI)、椎体前缘高度、矢状面Cobb角、并发症总发生率等指标。结果观察组病例手术用时缩短、透视次数、出血量以及其骨水泥注入量均少于对照组(P<0.05)。两组术前、术后1个月、术后3个月与术后6个月的ODI指数、术后6个月矢状面Cobb角、椎体前缘高度及并发症发生率比较,差异均无统计学意义(P>0.05)。结论单侧PVP治疗对患者腰椎功能、Cobb角、椎体前缘高度的改善效果与双侧PVP相当,但单侧PVP的创伤性更低,同时可减少透视次数、骨水泥注入量。Objective To apply unilateral percutaneous vertebroplasty(PVP)in the treatment of thoracolumbar compression fractures,and analyze its effect on lumbar function and therapeutic value.Methods A total of 86 patients with thoracolumbar compression fracture treated from June 2021 to June 2023 were randomly selected to be included in the study,and the patients were randomly divided into 2 groups,with 43 cases in each group.Both groups received PVP treatment.The observation group received unilateral puncture and the control group received bilateral puncture.The surgical indexes,Oswestry disability index(ODI),vertebral anterior margin height,sagittal Cobb Angle,and total complication rate were compared between the two groups.Results The time of operation,the number of fluoroscopy,the amount of blood loss and the amount of bone cement injection in the observation group were less than those in the control group(P<0.05).There were no significant differences in ODI index,sagittal Cobb Angle,vertebral anterior margin height and incidence of complications between the two groups before surgery,1 month after surgery,3 months after surgery and 6 months after surgery(P>0.05).Conclusions Unilateral PVP has the same improvement effect on lumbar function,Cobb Angle and vertebral anterior margin height as bilateral PVP,but unilateral PVP is less traumatic,and can reduce the number of fluoroscopy and the amount of bone cement injection,which has certain advantages.
