摘　　要：目的观察特定电磁波谱(TDP)治疗器联合硫辛酸与腺苷钴胺治疗老年2型糖尿病(T2DM)合并糖尿病周围神经病变(DPN)的临床效果。方法选取2021年1月—2022年12月六安市第二人民医院内分泌科收治的老年T2DM合并DPN患者100例,按照随机数字表法分为观察组和对照组,每组50例。对照组采用硫辛酸注射液+腺苷钴胺片治疗,观察组在对照组的基础上加用TDP治疗器对病变部位照射治疗,2组均治疗2周。比较2组临床疗效,治疗前后下肢正中神经和腓总神经的运动神经传导速度(MNCV)及感觉神经传导速度(SNCV)、神经病变程度评分,不良反应。结果观察组治疗总有效率高于对照组(100.00%vs.88.00%,χ^(2)=4.433,P=0.035)。治疗2周后,2组下肢正中神经和腓总神经的MNCV、SNCV高于治疗前,且观察组高于对照组(P<0.05或P<0.01);2组周围神经症状、下肢反射及足趾感觉评分低于治疗前,且观察组低于对照组(P<0.01)。2组治疗期间均无不良反应发生。结论TDP治疗器联合硫辛酸与腺苷钴胺治疗老年T2DM合并DPN的临床疗效确切,可有效缓解临床症状,提高神经传导速度,改善下肢功能,且安全性较高。Objective To observe the clinical efficacy of TDP combined with thioctic acid and cobamamide in the treatment of elderly T2DM complicated with DPN.Methods A total of 100 elderly patients with T2DM complicated with DPN who were admitted to the Department of Endocrinology of the Second People′s Hospital of Lu′an City from January 2021 to December 2022 were divided into the observation group and the control group according to the random number table method,with 50 cases in each group.The control group was treated with thioctic acid injection+cobamamide tablets,and the observation group was treated with TDP on the basis of the control group,and both groups were treated for 2 weeks.The clinical efficacy,the MNCV and SNCV of the median nerve and common peroneal nerve of the lower limb,neuropathy degree scores before and after treatment,and adverse reactions were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group(100.00%vs.88.00%,χ^(2)=4.433,P=0.035).After 2 weeks of treatment,the MNCV and SNCV of the median nerve and common peroneal nerve of the louer limb in the two groups were higher than those before treatment,and those in the observation group were higher than the control group(P<0.05 or P<0.01);The scores of peripheral nerve symptoms,lower limb reflexes and toe sensation in the two groups were lower than those before treatment,and the observation group were lower than those in the control group(P<0.01).There were no adverse reactions during the treatment period in the two groups.Conclusion TDP combined with thioctic acid and cobamamide in the treatment of elderly T2DM complicated with DPN has a definite clinical efficacy,which can effectively alleviate clinical symptoms,increase nerve conduction velocity,improve lower limb function,and with higher safety.
