钬激光碎石术治疗泌尿系结石的临床效果及对疼痛程度的影响

摘　　要：目的目的探讨钬激光碎石术治疗泌尿系结石的临床效果及对疼痛程度的影响。方法方法将2023年1月至2024年6月在我院治疗的118例泌尿系结石患者随机分为两组,对照组使用气压弹道碎石术,观察组使用输尿管软镜钬激光碎石术,对比两组的手术指标、并发症、肾功能及炎症指标。结果结果观察组手术时间、术中出血量、术后1dVAS疼痛评分少于对照组,一次碎石成功率高于对照组(P<0.05);与对照组相比,观察组在发热、血尿、肾绞痛及输尿管穿孔等并发症的发生率上均呈现较低水平(P<0.05);观察组术后3d的BUN、Scr、CRP、PCT均低于对照组(P<0.05)。结论结论钬激光碎石术治疗泌尿系结石的临床效果显著,手术创伤小,一次碎石成功率高,术后疼痛及炎症反应程度轻,对肾功能影响小,并发症发生率低。Objective To explore the clinical effects of holmium laser lithotripsy in the treatment of urinary calculi and its impact on pain levels.Method A total of 118 patients with urinary calculi treated at our hospital from January 2023 to June 2024 were randomly divided into two groups.The control group underwent pneumatic lithotripsy,while the observation group received ureteroscopic holmium laser litho-tripsy.Surgical indicators,complications,renal function,and inflammatory markers were compared between the two groups.Result The observation group had shorter operation times,less intraoperative blood loss,and lower postoperative 1-day VAS pain scores than the con-trol group,with a higher success rate of single-session lithotripsy(P<0.05).Compared with the control group,the observation group showed lower incidences of complications such as fever,hematuria,renal colic,and ureteral perforation(P<0.05).The observation group also had lower BUN,Scr,CRP,and PCT levels on postoperative day 3 compared to the control group(P<0.05).Conclusion Holmium laser litho-tripsy has significant clinical effects in the treatment of urinary calculi,with minimal surgical trauma,a high success rate of single-session lithotripsy,mild postoperative pain and inflammatory response,minimal impact on renal function,and a low incidence of complications.
