膝关节内侧副韧带损伤与骨挫伤及内侧半月板撕裂的相关性研究

摘　　要：目的 研究浅层内侧副韧带及半月板股骨韧带损伤与骨挫伤的相关性及半月板股骨韧带损伤与内侧半月板撕裂的相关性。方法 回顾性分析自2018-01—2023-06行膝关节MRI平扫检查的325例内侧副韧带损伤患者,根据MRI图像是否存在骨挫伤分为骨挫伤组(n=165)和无骨挫伤组(n=160),比较两组浅层内侧副韧带、半月板股骨韧带损伤情况的差异;根据MRI图像是否存在内侧半月板撕裂分为内侧半月板撕裂组(n=101)和无内侧半月板撕裂组(n=224),比较两组半月板股骨韧带损伤情况差异。结果 骨挫伤组和无骨挫伤组浅层内侧副韧带损伤情况差异无统计学意义(χ^(2)=1.327,P=0.249)。骨挫伤组半月板股骨韧带损伤发生率高于无骨挫伤组,差异有统计学意义(χ^(2)=44.977,P<0.001)。骨挫伤组与无骨挫伤组0级半月板股骨韧带损伤[χ^(2)=44.977,P<0.001,95%CI为0.132(0.069,0.252),Phi系数为-0.372]、Ⅰ级半月板股骨韧带损伤[χ^(2)=18.643,P<0.001,95%CI为0.147(0.055,0.392),Phi系数为-0.240]、Ⅲ级半月板股骨韧带损伤[χ^(2)=64.453,P<0.001,95%CI为7.314(4.390,12.183),Phi系数为0.445]情况差异有统计学意义且具有关联性。内侧半月板撕裂组的半月板股骨韧带损伤发生率较无内侧半月板撕裂组高,差异有统计学意义且具有关联性[χ^(2)=7.418,P=0.006,95%CI为2.378(1.259,4.491),Phi系数为0.151]。结论 半月板股骨韧带损伤与骨挫伤具有一定相关性,Ⅲ级半月板股骨韧带损伤与股骨内侧髁、股骨外侧髁骨挫伤有关,且骨挫伤在Ⅲ级半月板股骨韧带损伤时的发生率更高,约是无骨挫伤患者的7倍,内侧半月板体部、后角撕裂与半月板股骨韧带损伤具有一定相关性。Objective To investigate the correlation between superficial medial collateral ligament and meniscofemoral ligament injuries and bone bruises as well as the correlation between meniscofemoral ligament injuries and medial meniscus tears.Methods All 325 patients with medial collateral ligament injuries who underwent knee MRI examination from January 2018 to June 2023 were retrospectively analyzed and divided into the bone bruise group(n=165) and non-bone bruise group(n=160)according to the presence or absence of bone bruises in the MRI images to compare the differences in the superficial medial collateral ligament and meniscofemoral ligament injuries between two groups.In addition,the patients were also divided into the medial meniscus tear group(n=101) and a non-medial meniscus tear group(n=224) according to the presence or absence of medial meniscus tears on MRI images to compare the difference in meniscofemoral ligament injuries between two groups.Results The difference in superficial medial collateral ligament injuries between the bone bruise group and non-bone bruise group was not statistically significant(χ^(2)=1.327,P=0.249).The incidence of meniscofemoral ligament injuries in the bone bruise group was higher than that in the non-bone bruise group,with a statistically significant difference(χ^(2)=44.977,P<0.001).The difference between grade 0 meniscofemoral ligament injuries [χ^(2)=44.977,P<0.001,95%CI 0.132(0.069,0.252),Phi coefficient-0.372],grade I meniscofemoral ligament injuries [χ^(2)=18.643,P<0.001,95%CI 0.147(0.055,0.392),Phi coefficient-0.372] and grade Ⅲ meniscofemoral ligament injuries [χ^(2)=64.453,P<0.001,95%CI 7.314(4.390,12.183),Phi coefficient 0.445] was statistically significant and correlated.The incidence of meniscofemoral ligament injuries in the medial meniscus tear group was higher than that a non-medial meniscus tear group,with statistically significant difference and correlation [χ^(2)=7.418,P=0.006,95%CI 2.378(1.259,4.491),Phi coefficient 0.151].Conclusion Meniscofemoral liga
