摘 要:目的 应用身高测量法与下肢长度测量法对前交叉韧带重建患者移植物直径进行术前预测,比较两种预测方法的准确性。方法 纳入自2022-06—2023-10行膝关节镜下自体腘绳肌腱单束前交叉韧带重建手术治疗的190例膝关节前交叉韧带损伤患者,记录患者年龄、身高、下肢长度及术中测量的移植物直径。将190例分为预测组(n=70)和验证组(n=120),对预测组中身高、下肢长度与移植物直径进行相关性分析,对强相关指标建立移植物直径预测方程,然后将验证组数据代入方程,计算预测值与术中测量值的误差,比较两种预测方法的准确性。结果 预测组的身高、下肢长度、性别、移植物直径与验证组差异无统计学意义(P>0.05)。预测组身高与移植物直径呈强相关,相关系数r=0.741,身高测量法的预测方程为:移植物直径(mm)=4.32×身高(m)+0.38。下肢长度与移植物直径呈强相关,相关系数r=0.695,下肢长度预测法的预测方程为:移植物直径(mm)=5.53×下肢长度(m)+2.41。身高测量法预测值为(7.758±0.394)mm,下肢长度测量法预测值为(7.730±0.325)mm;用术中测量的移植物直径减去预测值,将其差值的绝对值定义为预测误差,身高测量法误差值为(0.293±0.219)mm,下肢长度测量法误差值为(0.289±0.216)mm。验证组身高测量法与下肢长度测量法的移植物直径预测值(t=0.580,P=0.562)、误差值(t=0.161,P=0.872)差异均无统计学意义。结论 身高、下肢长度与前交叉韧带重建患者移植物直径均存在强相关关系,身高测量法和下肢长度测量法均可对患者移植物直径进行术前预测,两种方法误差无显著差异。Objective To predict graft diameter in anterior cruciate ligament reconstruction patients using height and lower limb length,and compare the accuracy of both methods.Methods A total of 190 patients who sustained an anterior cruciate ligament injury and underwent arthroscopic single-bundle anterior cruciate ligament reconstruction using autologous hamstring tendon grafts between June 2022 and October 2023 were enrolled.The patient age,height,lower limb length,and the intraoperatively measured graft diameter were documented.One hundred ninety patients were divided into a prediction group(n=70)and a validation group(n=120).Correlation analysis was conducted onheight,lower limb length,and graft diameter among patients in the prediction group.Separate regression equations for graft diameter were calculated based on height and lower limb length.Results No statistically significant differences were observed in height,lower limb length,sex,and graft diameter between the prediction and validation cohorts(P>0.05).A strong correlation(r=0.741) was found between height and graft diameter in the prediction group;the predictive equation was graft diameter(mm)=4.32×Height(m)+0.38.Lower limb length also demonstrated a strong correlation with graft diameter(r=0.695).The predictive equation based on lower limb length was:graft diameter(mm)=5.53×Lower limb length(m)+2.41.Predictions of graft diameter using patient height yielded a result of(7.758±0.394)mm,whereas predictions using patient lower limb length resulted in(7.730±0.325) mm.Predictive errors,defined as the absolute difference between measured and predicted graft diameters,were(0.293±0.219) mm for height-based predictions and(0.289±0.216) mm for lower limb length-based predictions.Statistical analysis showed no significant differences in predicted(t=0.580,P=0.562) or error(t=0.161,P=0.872) values between the height and lower limb length methods for graft diameter prediction.Conclusion Height and lower limb length exhibit strong correlations with graft diameter in ant
