摘 要:目的探究越顶(OTT)技术重建前交叉韧带(ACL)术后移植物应力分布特征。方法基于膝关节CT及MRI影像学数据,建立膝关节有限元模型。模型应用线弹性材料属性。骨的材料属性设置为弹性模量为12000.0 MPa,泊松比为0.3;韧带及移植物材料属性设置为弹性模量390.0 MPa,泊松比为0.4。将股骨固定设置为模型边界条件,对胫骨施加前向134 N拉力作为载荷条件。分析正常ACL及OTT技术重建术后移植物的拉应力、压应力、剪切应力和等效应力的受力特征。结果OTT技术重建ACL后移植物拉应力最大值(0.9 MPa)在股骨端前侧,正常ACL拉应力最大值(2.5 MPa)在股骨端后侧。压应力最大值(4.1 MPa)、剪切应力最大值(2.8 MPa)、等效应力最大值(3.2 MPa)均在股骨端前侧,与正常ACL压应力最大值(4.6 MPa)、剪切应力最大值(3.6 MPa)、等效应力最大值(4.7 MPa)位置一致。结论OTT技术重建ACL术后移植物压应力、剪切应力及等效应力最大值均在股骨端前侧,与正常ACL位置一致。Objective To investigate the stress distribution characteristics of grafts after over-the-top(OTT)reconstruction on anterior cruciate ligament(ACL).Methods A finite element model of the knee joint was established based on CT and MRI imaging data.Linear elastic material properties were applied to the model.The elastic modulus of bone was set to 12000.0 MPa,and the Poisson's ratio was 0.3.The material properties of ligament and graft were set as elastic modulus 390.0 MPa and Poisson's ratio 0.4.The femur was fixed as the boundary condition of the model;and the tibia was applied with 134 N forward tension as the loading condition.The stress characteristics of tensile stress,compressive stress,shear stress and equivalent stress of normal ACL reconstruction and OTT reconstruction grafts were analyzed.Results The maximum tensile stress of OTT reconstruction ACL posterior graft(0.9 MPa)was in the anterior femoral end,and the maximum tensile stress of normal ACL(2.5 MPa)was in the posterior femoral end.The maximum compressive stress(4.1 MPa),the maximum shear stress(2.8 MPa);and the maximum equivalent stress(3.2 MPa)were all in the front of the femoral end.The locations of the maximum compressive stress(4.6 MPa),shear stress(3.6 MPa),and equivalent stress(4.7 MPa)of the normal ACL were consistent.Conclusion The maximum compressive stress,shear stress,and equivalent stress of the graft after ACL reconstruction with OTT were all located in the anterior femoral end,which was consistent with the position of the normal ACL.
