摘 要:目的对比研究3D打印数字化塑形聚醚醚酮(PEEK)与钛网修补颅骨缺损的效果。方法回顾性选取2021年1月至2023年9月解放军总医院第九医学中心收治的颅骨缺损患者78例,根据应用修补材料的不同分为PEEK组(采用3D打印数字化塑形PEEK修补,n=41)和钛网组(采用钛网修补,n=37)。对比两组手术时间、术中出血量、住院时间、并发症、塑形满意度、预后情况[格拉斯哥预后评分(GOS)、简易精神状态评价量表(MMSE)评分、Karnofsky功能状态评分(KPS)]。结果PEEK组和钛网组术中出血量及住院时间比较差异均无统计学意义(P>0.05),但与钛网组比较,PEEK组手术时间更长(P<0.05)。PEEK组总并发症发生率与钛网组比较差异无统计学意义(P>0.05)。PEEK组塑形满意度较钛网组更高(P<0.05)。术前PEEK组和钛网组GOS、MMSE、KPS评分比较差异均无统计学意义(P>0.05),术后两组GOS、MMSE、KPS评分均升高,且与钛网组比较,PEEK组患者上述评分均更高(P<0.05)。结论在颅骨缺损患者中应用3D打印数字化塑形PEEK与钛网颅骨修补术的并发症相当,但与钛网相比,PEEK塑形效果更佳,患者预后更好。Objective To compare the effects of 3D printed digitally shaped polyetheretherketone(PEEK)and titanium mesh in repairing skull defects.Methods A total of 78 patients with skull defects admitted to the Ninth Medical Centre of the Chinese PLA General Hospital between January 2021 and September 2023 were retrospectively enrolled.According to different repair materials,they were divided into 3D PEEK group(n=41)and titanium mesh group(n=37).The operation time,intraoperative blood loss,length of hospital stay,complications,shaping satisfaction and prognosis[Glasgow prognostic scale(GOS),mini-mental state examination(MMSE),Karnofsky performance status(KPS)]were compared between the two groups.Results There was no significant difference in intraoperative blood loss or length of hospital stay between the two groups(P>0.05),but the operation time in PEEK group was longer than that in titanium mesh group(P<0.05).There was no significant difference in total incidence of complications between the two groups(P>0.05).The shaping satisfaction in PEEK group was higher than that in titanium mesh group(P<0.05).Before the surgery,there was no significant difference in scores of GOS,MMSE and KPS between the two groups(P>0.05),but after the surgery,scores of GOS,MMSE and KPS were all increased in both groups,with PEEK group higher than titanium mesh group(P<0.05).Conclusions The repair effect of 3D printed digitally shaped PEEK and titanium mesh is almost the same in patients with skull defects,but compared to titanium mesh,3D PEEK has better shaping effects and prognosis.
