依替米星联合三代头孢菌素治疗老年社区获得性肺炎的临床效果

摘　　要：目的:分析依替米星联合三代头孢菌素治疗老年社区获得性肺炎的临床效果。方法:选取2022年6月—2024年2月邳州市中医院收治的76例老年社区获得性肺炎患者为研究对象,采用随机数字表法分为对照组和观察组,各38例。对照组采用三代头孢菌素治疗,观察组在对照组基础上联合依替米星治疗。对比两组炎性指标、症状消失时间、肺功能、不良反应发生情况。结果:治疗10 d后,两组C反应蛋白、降钙素原、肿瘤坏死因子-α水平及白细胞计数降低,观察组低于对照组(P<0.05);观察组发热、寒战、胸痛、咳嗽、咳痰消失时间均短于对照组(P<0.05);治疗5、10 d后,两组第1秒用力呼气容积、用力肺活量、呼气流量峰值水平升高,观察组高于对照组(P<0.05);两组不良反应总发生率对比,无统计学差异(P>0.05)。结论:依替米星联合三代头孢菌素治疗老年社区获得性肺炎效果良好,可减轻炎症,加快恢复速度,改善肺功能,且未增加不良反应发生风险。Objective:To analyze the clinical effect of Etimicin combined with third-generation cephalosporins in the treatment of elderly community-acquired pneumonia.Methods:A total of seventy-six elderly patients with community-acquired pneumonia admitted to Pizhou Hospital of Traditional Chinese Medicine from June 2022 to February 2024 were selected as the study objects.They were divided into control group and observation group by random number table method,with thirty-eight cases in each group.The control group was treated with third-generation cephalosporins,and the observation group was treated with Etimicin on the basis of the control group.The inflammatory indexes,time of symptom disappearance,lung function and occurrence of adverse reactions were compared between the two groups.Results:After 10 days of treatment,the levels of C-reactive protein,procalcitonin,tumor necrosis factor-αand white blood cell count in both groups were decreased,and the observation group was lower than the control group(P<0.05);the disappearance time of fever,chills,chest pain,cough and expectoration in observation group was shorter than that in control group(P<0.05);after 5 and 10 days of treatment,the peak levels of forced expiratory volume,forced vital capacity and expiratory flow in the first second of the two groups were increased,and the observation group was higher than the control group(P<0.05);there was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Etimicin combined with third-generation cephalosporins has a good effect in the treatment of community-acquired pneumonia in the elderly,which can reduce inflammation,accelerate recovery and improve lung function without increasing the risk of adverse reactions.
