腹型肥胖患者腹腔内脏脂肪面积与胰岛素抵抗指数的相关性分析

摘　　要：目的:分析腹型肥胖患者腹腔内脏脂肪面积(VFA)与胰岛素抵抗指数(HOMA-IR)的相关性。方法:选取2022年1月—2024年5月在广东医科大学附属第二医院接受诊疗的腹型肥胖患者66例为试验组,另选取同期在广东医科大学附属第二医院接受健康体检的非腹型肥胖志愿者66例为对照组。测量所有入组者VFA、血清空腹胰岛素(FIN)、空腹血糖(FBG),并计算HOMA-IR。比较两组腹腔VFA、血清FIN、FBG水平及HOMA-IR。根据VFA水平将分为低VFA组、中VFA组、高VFA组,比较三组血清FIN、FBG水平及HOMA-IR。采用Spearman相关系数分析试验组腹腔VFA与HOMA-IR的相关性。结果:试验组腹腔VFA、血清FIN、FBG水平及HOMA-IR高于对照组(P<0.05)。高VFA组血清FIN水平及HOMA-IR高于低VFA组与中VFA组,中VFA组血清FIN水平及HOMA-IR高于低VFA组(P<0.05);三组FBG水平比较,差异无统计学意义(P>0.05)。Spearman相关性分析结果显示,试验组腹腔VFA与HOMA-IR呈正相关(r=0.621,P<0.001)。结论:腹型肥胖患者腹腔VFA与HOMA-IR呈正相关,早期测量VFA有利于评估其胰岛功能。Objective:To analyze the correlation between abdominal visceral fat area(VFA)and insulin resistance index(HOMAIR)in patients with abdominal obesity.Methods:A total of sixty-six patients with abdominal obesity who received medical treatment in the Second Affiliated Hospital of Guangdong Medical University from January 2022 to May 2024 were selected as the experimental group,and sixty-six volunteers with non-abdominal obesity who received physical examination in the Second Affiliated Hospital of Guangdong Medical University during the same period were selected as the control group.VFA,fasting insulin(FIN)and fasting blood glucose(FBG)were measured in all participants,and HOMA-IR was calculated.The levels of abdominal VFA,serum FIN,FBG and HOMA-IR were compared between the two groups.According to the VFA level,the patients were divided into low VFA group,medium VFA group and high VFA group,and the serum FIN,FBG and HOMA-IR levels of the three groups were compared.Spearman correlation coefficient was used to analyze the correlation between intraperitoneal VFA and HOMA-IR.Results:The levels of intraperitoneal VFA,serum FIN,FBG and HOMA-IR in experimental group were higher than those in control group(P<0.05).Serum FIN level and HOMA-IR in high VFA group were higher than those in low VFA group and medium VFA group,and serum FIN level and HOMA-IR in medium VFA group were higher than those in low VFA group(P<0.05);there was no significant difference in FBG level among the three groups(P>0.05).Spearman correlation analysis showed that there was a positive correlation between peritoneal VFA and HOMA-IR(r=0.621,P<0.001).Conclusion:There is a positive correlation between abdominal VFA and HOMA-IR in patients with abdominal obesity,and early measurement of VFA is beneficial to assess islet function.
