超重或肥胖的初诊2型糖尿病患者应用司美格鲁肽联合二甲双胍治疗的临床效果

摘　　要：目的:观察超重或肥胖的初诊2型糖尿病(T2DM)患者应用司美格鲁肽联合二甲双胍治疗的临床效果。方法:选取2023年1—8月苏州永鼎医院收治的102例初诊T2DM伴超重或肥胖患者为研究对象,通过随机数字表法分为对照组和观察组,各51例。对照组口服盐酸二甲双胍片,观察组在对照组基础上皮下注射司美格鲁肽。比较两组空腹血糖(FBG)、餐后2 h血糖(2 hPBG)、糖化血红蛋白(HbA1c)、胰岛素抵抗指数(HOMA-IR)、身体质量指数(BMI)、甘油三酯(TG)、胆固醇(TC)水平,不良反应发生情况及临床疗效。结果:治疗前,两组FBG、2 hPBG、HbA1c、HOMA-IR水平比较,差异无统计学意义(P>0.05);治疗后,两组FBG、2 hPBG、HbA1c、HOMA-IR水平降低,观察组低于对照组(P<0.05)。治疗前,两组BMI、TG、TC水平比较,差异无统计学意义(P>0.05);治疗后,两组BMI、TG、TC水平降低,观察组低于对照组(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。观察组治疗总有效率高于对照组(P=0.046)。结论:超重或肥胖的初诊T2DM患者应用司美格鲁肽联合二甲双胍治疗的临床效果确切,可改善患者血糖、血脂指标,降低BMI,且未增加不良反应发生风险。Objective:To observe the clinical effect of semiglutide combined with metformin in newly diagnosed type 2 diabetes mellitus(T2DM)patients who were overweight or obese.Methods:A total of one hundred and two newly diagnosed T2DM patients with overweight or obese admitted to Suzhou Yongding Hospital from January to August 2023 were selected as the study objects,and were divided into control group and observation group by random number table method,with fifty-one cases in each group.The control group took metformin hydrochloride tablets orally,and the observation group was injected with semiglutide under the basal epithelium of the control group.Fasting blood glucose(FBG),2 h postprandional blood glucose(2 hPBG),glycated hemoglobin(HbA1c),insulin resistance index(HOMA-IR),body mass index(BMI),triglyceride(TG),cholesterol(TC)levels,occurrence of adverse reactions and clinical efficacy of the two groups were compared.Results:Before treatment,there was no significant difference in FBG,2 hPBG,HbA1c and HOMA-IR levels between the two groups(P>0.05);after treatment,the levels of FBG,2 hPBG,HbA1c and HOMA-IR in both groups were decreased,and the observation group was lower than the control group(P<0.05).Before treatment,there was no significant difference in BMI,TG and TC levels between the two groups(P>0.05);after treatment,the levels of BMI,TG and TC in both groups were decreased,and the observation group was lower than the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).The total effective rate of observation group was higher than that of control group(P=0.046).Conclusion:In newly diagnosed T2DM patients who are overweight or obese,semiglutide combined with metformin has a definite clinical effect,which can improve blood glucose and lipid indexes and reduce BMI without increasing the risk of adverse reactions.
