摘　　要：目的:探讨血小板抗体检测联合交叉配型对输注血小板患者凝血功能及安全性的影响。方法:选取2021年5月—2024年5月于常州市第一人民医院输注血小板的60例患者为研究对象,根据随机数字表法分为两组,对照组(30例)采取常规输注方法;试验组(30例)先进行血小板抗体检测联合交叉配型,再行血小板输注。对比两组血小板计数增加指数(CCI)、输注有效率、凝血功能[活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、凝血酶时间(TT)]、输血相关并发症发生情况。结果:输注后1 h、24 h,试验组CCI均大于对照组(P<0.001);试验组输注有效率高于对照组(P=0.023);输注后24 h,两组APTT、PT、TT缩短,试验组短于对照组(P<0.05);试验组输血相关并发症总发生率低于对照组(P=0.011)。结论:血小板抗体检测联合交叉配型可提高血小板输注效果,改善患者凝血功能,且安全性较高。Objective:To investigate the effect of platelet antibody detection combined with cross-matching on coagulation function and safety in patients with platelet transfusion.Methods:Sixty patients receiving platelet transfusion in Changzhou First People's Hospital from May 2021 to May 2024 were selected as the study objects and divided into two groups according to random number table method.The control group(30 cases)received conventional infusion,and the experimental group(30 cases)received platelet antibody detection combined with cross-matching before platelet infusion.The increase index of platelet count(CCI),the effective rate of infusion,coagulation function[activated partial thromboplastin time(APTT),prothrombin time(PT),thrombin time(TT)]and the incidence of transfusion related complications were compared between the two groups.Results:At 1 h and 24 h after infusion,CCI in experimental group was higher than that in control group(P<0.001);the effective rate of infusion in experimental group was higher than that in control group(P=0.023);24 h after infusion,APTT,PT and TT were shortened in both groups,and those in experimental group were shorter than those in control group(P<0.05);the total incidence of transfusion-related complications in experimental group was lower than that in control group(P=0.011).Conclusion:Platelet antibody detection combined with cross-matching can improve the platelet transfusion effect,improve the coagulation function of patients,and have high safety.
