术中肋间神经阻滞与术前超声引导下椎旁阻滞在胸腔镜肺部手术中的应用效果分析

摘　　要：目的:分析术中肋间神经阻滞(ICNB)与术前超声引导下椎旁阻滞(PVB)在胸腔镜肺部手术中的应用效果。方法:将2023年1—12月于兴化市人民医院行胸腔镜肺部手术的60例患者纳入研究,以随机数字表法分为两组,观察组(30例)行术前超声引导下PVB,对照组(30例)行术中ICNB。比较两组手术相关情况、术后疼痛情况及并发症发生情况。结果:观察组术后首次下床时间早于对照组(P<0.001);两组手术时间、总出血量、术后引流时间、住院时间比较,无统计学差异(P>0.05)。观察组术后2、6、12、24 h静息和咳嗽状态下视觉模拟评分法评分均低于对照组(P<0.05)。两组术后并发症发生率比较,无统计学差异(P>0.05)。结论:与术中ICNB相比,术前超声引导下PVB可缩短术后首次下床时间,缓解术后疼痛,且不会增加并发症发生风险。Objective:To analyze the effects of intraoperative intercostal nerve block(ICNB)and ultrasound-guided paravertebral block(PVB)in thoracoscopic lung surgery.Methods:A total of sixty patients undergoing thoracoscopic lung surgery in Xinghua People's Hospital from January to December 2023 were included in the study and divided into two groups by random number table method.The observation group(30 cases)received PVB guided by ultrasound before surgery,and the control group(30 cases)received ICNB during surgery.The operation related conditions,postoperative pain and complications were compared between the two groups.Results:The first time of getting out of bed in observation group was earlier than that in control group(P<0.001);there were no significant differences in operation time,total blood loss,postoperative drainage time and hospital stay between the two groups(P>0.05).The visual analog score of the observation group was lower than that of the control group at 2,6,12,and 24 hours after surgery in both resting and coughing states(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion:Compared with intraoperative ICNB,preoperative ultrasound-guided PVB can shorten the first time of getting out of bed after surgery,relieve postoperative pain,and do not increase the risk of complications.
