综合护理预防肛周脓肿患者术后排尿障碍的效果分析

摘　　要：目的:分析综合护理预防肛周脓肿患者术后排尿障碍的效果。方法:选取2022年11月—2024年6月于滨州医学院附属医院行肛周脓肿手术治疗的患者83例为研究对象,随机分为两组。对照组(41例)行常规护理,试验组(42例)在对照组基础上行综合护理。对比两组自主排尿功能恢复情况、尿动力学指标、肛周创面疼痛情况、生活质量。结果:试验组首次自主排尿恢复时间短于对照组,首次自主排尿后膀胱残余尿量少于对照组,首次自主排尿后2 h内排尿量大于对照组(P<0.001);护理后,两组最大尿流率、最大逼尿肌压力增大,膀胱初感容积缩小,试验组优于对照组(P<0.05);护理3、5 d后,两组数字评分法评分降低,试验组低于对照组(P<0.05);护理后,两组生理机能、生理职能、情感职能评分升高,试验组高于对照组(P<0.05)。结论:综合护理预防肛周脓肿患者术后排尿障碍的效果显著,可促进患者自主排尿功能恢复,改善尿动力学指标,缓解肛周疼痛,提高术后生活质量。Objective:To analyze the effect of comprehensive nursing on preventing postoperative voiding disorder in patients with perianal abscess.Methods:A total of eighty-three patients who underwent perianal abscess surgery in the Affiliated Hospital of Binzhou Medical College from November 2022 to June 2024 were selected as the study objects and randomly divided into two groups.The control group(41 cases)received routine nursing,the experimental group(42 cases)received comprehensive nursing on the basis of the control group.The recovery of autonomic urination function,urodynamic indexes,perianal wound pain and quality of life were compared between the two groups.Results:The recovery time of the first spontaneous urination in experimental group was shorter than that in control group,the residual urine volume after the first spontaneous urination was less than that in control group,and the urine volume within 2 h after the first spontaneous urination was greater than that in control group(P<0.001);after nursing,the maximum urine flow rate and the maximum detrusor pressure increased,and the bladder initial sensing volume decreased,and the experimental group was better than the control group(P<0.05);after 3 and 5 days of nursing,the scores of the two groups by digital scoring method were decreased,and the experimental group was lower than the control group(P<0.05);after nursing,the scores of physiological function,physiological function and emotional function of the two groups were increased,and the experimental group was higher than the control group(P<0.05).Conclusion:Comprehensive nursing can significantly prevent postoperative voiding disorders in patients with perianal abscess,promote the recovery of patients'autonomous voiding function,improve urodynamic indexes,relieve perianal pain,and improve postoperative quality of life.
