摘 要:目的:探讨关节镜辅助下微创经皮钢板固定(MIPPO)技术结合锁定钢板治疗SchatzkerⅢ型胫骨平台骨折(TPF)的临床效果。方法:按随机数字表法将2020年1月—2023年1月瑞昌市人民医院收治的80例SchatzkerⅢ型TPF分为两组,各40例,对照组给予锁定钢板内固定治疗,联合组给予关节镜辅助下MIPPO技术结合锁定钢板内固定治疗。对比两组并发症发生情况、炎症反应[白介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)]、恢复情况、关节功能[美国足与踝关节协会(AOFAS)踝-后足评分系统评分、美国特种外科医院(HSS)膝关节评分]。结果:与对照组相比,联合组并发症发生率低,差异有统计学意义(P<0.05)。术前两组IL-1β、TNF-α、CRP水平比较,差异均无统计学意义(P>0.05);与对照组相比,联合组术后IL-1β、TNF-α、CRP水平均低,差异均有统计学意义(P<0.05)。与对照组相比,联合组骨折愈合时间、负重时间均早,差异均有统计学意义(P<0.05)。术前两组AOFAS踝-后足评分系统、HSS膝关节评分比较,差异均无统计学意义(P>0.05),与对照组相比,联合组术后AOFAS踝-后足评分系统、HSS膝关节评分均高,差异均有统计学意义(P<0.05)。结论:关节镜辅助下MIPPO技术结合锁定钢板治疗SchatzkerⅢ型TPF利于减少术后并发症发生,降低炎症反应,促进骨折愈合,缩短负重时间,提升踝关节、膝关节功能。Objective:To investigate the clinical effect of arthroscopy-assisted minimally invasive percutaneous plate osteosynthesis(MIPPO)technology combined with locking plate in the treatment of Schatzker typeⅢtibial plateau fracture(TPF).Method:According to random number table method,80 cases of Schatzker typeⅢTPF treated in Ruichang People's Hospital from January 2020 to January 2023 were divided into two groups,with 40 cases in each group.The control group was treated with locked plate internal fixation,and the combined group was treated with arthroscopy-assisted MIPPO technology combined with locked plate internal fixation.Complications,inflammation[interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α),C reactive protein(CRP)],recovery,and joint function[American foot and ankle association(AOFAS)ankle hindfoot scale score,hospital for special surgery(HSS)knee joint score]were compared between the two groups.Result:Compared with the control group,the complication rate of the combined group was lower,the difference was statistically significant(P<0.05).There were no statistically significant differences in IL-1β,TNF-αand CRP between the two groups before surgery(P>0.05),and the levels of IL-1β,TNF-αand CRP in the combined group were lower than those in the control group after surgery(P<0.05).Compared with the control group,the fracture healing time and the weightbearing time of the combined group were earlier,the differences were statistically significant(P<0.05).There were no statistically significant differences in AOFAS ankle hindfoot scale and HSS knee joint scores between the two groups before surgery(P>0.05),and compared with control group,AOFAS ankle hindfoot scale and HSS knee joint scores in combined group were higher after surgery,the differences were statistically significant(P<0.05).Conclusion:Arthroscopy-assisted MIPPO technology combined with locking plate in the treatment of Schatzker typeⅢTPF is beneficial to reduce postoperative complications,reduce inflammation,promote fracture healing,sh
