新生儿低血糖症的临床特点及早期监测的临床研究

摘　　要：目的探讨新生儿低血糖症(NH)的临床特点及早期监测的重要性,为NH规范化管理提供临床参考依据。方法通过对2020年7月至2023年6月于柳州市中医医院住院的新生儿1201例进行血糖监测,按照血糖水平分为低血糖组(92例)和正常血糖组(1109例),建立一个研究队列,采集两组新生儿基本信息、临床特点及转归,并进行统计分析。结果与正常血糖组比较,低血糖组的临床特点在吃奶少、低热、抖动、抽搐方面较明显,多合并有早产、小于胎龄儿、高胆红素血症及母亲合并妊娠糖尿病,差异有统计学意义(P<0.05);NH发生的高峰期在出生后0~1 h内。结论NH临床症状无特异性,主要表现为吃奶少、低热、抖动、抽搐;合并有早产、小于胎龄儿、高胆红素血症,母亲合并妊娠糖尿病多见;发生的高峰期在出生后约1 h内,因此早监测、早发现、早干预血糖非常必要。并发新生儿低血糖性脑病及后遗症等新生儿少,转归良好。Objective To explore the clinical characteristics and monitoring importance of neonatal hypoglycemia(NH),and provide clinical reference for standardized management of NH.Methods By monitoring the blood glucose of 1201 neonates hospitalized in Liuzhou Traditional Chinese Medical Hospital from July 2020 to June 2023,and dividing them into a hypoglycemic group(92 cases)and a normoglycemic group(1109 cases)according to their blood glucose levels,a study cohort was established,and the basic information,clinical characteristics,and regressions of the two groups of neonates were collected and statistically analyzed.Results Compared with the normoglycemic group,the clinical features of the hypoglycemic group were more pronounced in terms of low milk intake,low-grade fever,shaking,and convulsions,combined with preterm labor,babies younger than the gestational age,hypercholesterolemia,and gestational diabetes mellitus in mothers,with statistically significant differences(P<0.05).The peak incidence of NH occurs within 0 to 1 hour after birth.Conclusion NH clinical symptoms are not specific,the main manifestations are low milk intake,low fever,shaking,convulsions;combined with preterm labor,less than the fetal age of the child,hypercholesterolemia and mother combined with gestational diabetes mellitus is common.The peak of the occurrence of the postnatal period of about 1 h,so early monitoring,early detection,early intervention of blood glucose is very necessary.Complications such as neonatal hypoglycemic encephalopathy and sequelae are less common,and the prognosis is generally good.
