心力衰竭合并心室重构患者血清胱抑素C、N-末端脑钠肽前体、心肌酶谱的临床诊断价值

摘　　要：目的探讨血清胱抑素C(CysC)、N-末端脑钠肽前体(NT-proBNP)、心肌酶谱在心力衰竭合并心室重构患者中的临床诊断价值。方法选取南方医科大学顺德医院附属杏坛医院于2023年1—12月内一科收治的临床诊断为心力衰竭患者为心衰组(n=50)、心力衰竭合并心室重构患者为重构组(n=50);入组同期南方医科大学顺德医院附属杏坛医院体检中心基线资料匹配的健康人群为对照组(n=50)。检测入组患者空腹血清中CysC、NT-proBNP、心肌酶谱的水平;完善彩色多普勒超声心动图检查,测量心室重构指标等。结果三组血清CysC、NT-proBNP、心肌酶谱(肌酸激酶、α羟基丁酸脱氢酶、肌酸激酶同工酶和乳酸脱氢酶)比较,差异有统计学意义(P<0.05);CysC+NT-proBNP+心肌酶谱联合检测心力衰竭合并心室重构阳性率为90.00%,高于单一指标检测的阳性率,差异有统计学意义(P<0.05);三组心室重构指标比较,差异有统计学意义(P<0.05)。结论CysC、NT-proBNP、心肌酶谱对心力衰竭合并心室重构患者具有较高的临床诊断价值,对患者的病情、预后、治疗方案的选择有重要的指导意义。Objective To explore the clinical diagnostic value of serum cystatin C(CysC),N-terminal pro-B-type natriuretic peptide(NT-proBNP)and myocardial enzyme spectrum in patients with heart failure complicated with ventricular remodeling.Methods The patients diagnosed as heart failure in the Department of Internal MedicineⅠof Xingtan Hospital Affiliated of Southern Medical University Shunde Hospital from January to December 2023 were selected as the heart failure group(n=50)and the patients with heart failure complicated with ventricular remodeling group(n=50),and the healthy population matched with baseline data from the physical examination center of Xingtan Hospital Affiliated of Southern Medical University Shunde Hospital during the same period were selected as the control group(n=50).The levels of CysC,NT-proBNP and myocardial enzyme spectrum in fasting serum were detected.The color Doppler echocardiography was improved and ventricular remodeling indexes were measured.Results There were statistically significant differences(P<0.05)in the serum CysC,NT proBNP,and myocardial enzyme spectrum(creatine kinase,alpha hydroxybutyrate dehydrogenase,creatine kinase isoenzyme,and lactate dehydrogenase)among the three groups.The positive rate of CysC+NT-proBNP+myocardial enzyme spectrum combined detection of heart failure complicated with remodeling was 90.00%,which was higher than that of single index detection,and the difference was statistically significant(P<0.05).The indexes of ventricular remodeling in the three groups of patients were significantly different(P<0.05).Conclusion CysC,NT proBNP,and myocardial enzyme spectrum have high clinical diagnostic value for patients with heart failure complicated with ventricular remodeling,and have important guiding significance for the patient’s condition,prognosis,and treatment plan selection.
