肺结核合并糖尿病患者的临床特点及淋巴细胞亚群分析

摘　　要：目的探讨肺结核合并糖尿病(PTB-DM)患者的临床特点及免疫功能状况。方法回顾性分析2020年1月至2022年6月82例PTB-DM患者(PTB-DM组)和同期470例单纯肺结核患者(PTB组)的临床资料,比较两组患者性别、年龄、临床症状、C反应蛋白(CRP)、胸部CT病灶范围及空洞率、痰结核菌阳性率、淋巴细胞亚群(CD4^(+)和CD8^(+)百分比、计数及比值)及临床疗效等,并分析PTB-DM的危险因素。结果两组年龄、临床症状、C反应蛋白、空洞率、痰结核菌阳性率、CD4^(+)计数、CD8^(+)计数与临床疗效比较,差异有统计学意义(P<0.05)。Logistic回归分析显示年龄及CD4^(+)计数是PTB-DM的危险因素(P<0.05)。结论PTB-DM患者患病年龄大,临床症状及炎症反应更明显,痰结核分枝杆菌涂片阳性率更高,易合并空洞,且其免疫功能受损,抗结核疗效差。年龄和CD4^(+)计数是PTB-DM患者患病的危险因素,临床应重视PTB-DM患者的诊断和治疗。Objective To investigate the clinical characteristics and immune function of patients with pulmonary tuberculosis complicated with diabetes mellitus(PTB-DM).Methods The clinical data of 82 patients with PTB-DM(the PTB-DM group)and 470 patients with simple pulmonary tuberculosis(the PTB group)admitted to our hospital from January 2020 to June 2022 were retrospectively analyzed.The gender,age,clinical symptoms,Creactive protein(CRP),chest CT lesion range and cavity rate,sputum positive rate,lymphocyte subsets(CD4^(+)and CD8^(+)percentage,count and ratio)and clinical efficacy were compared between the two groups,and the risk factors of PTB-DM were analyzed.Results There were significant differences in age,clinical symptoms,CRP,cavity rate,sputum positive rate,CD4^(+)count,CD8^(+)count and clinical efficacy between the two groups(P<0.05).Logistic regression analysis showed that age and CD4^(+)count were risk factors for PTB-DM(P<0.05).Conclusion PTB-DM patients are older,with more obvious clinical symptoms and inflammatory response,higher positive rate of sputum Mycobacterium tuberculosis smear,more likely to be complicated with cavity,impaired immune function,and poor anti-tuberculosis efficacy.Age and CD4^(+)count are the risk factors of PTB-DM.More attention should be paid to the detection and treatment of PTB-DM patients.
