摘　　要：目的:探讨16cm容积扫描模式在房颤患者左心房肺静脉血管成像检查中的应用价值。方法:选取60例射频消融术前行心脏CT血管成像(CTA)扫描的心房颤动患者,随机分为A组、B组。A组采用16cm容积扫描模式(管电压100kV,开启智能管电流技术);B组采用心电门控螺旋扫描模式,其余扫描参数同A组。2组患者扫描范围均为气管隆突上2cm至心脏隔底。对2组患者图像质量、剂量长度乘积(DLP)、有效剂量(ED)等进行统计学分析。结果:当心率>75次/min时,A组图像质量高于B组,差异有统计学意义(P<0.05);当心率≤75次/min时,2组图像质量差异没有统计学意义。A组的辐射剂量为(4.01±2.82)mSv,B组的辐射剂量为(8.51±0.60)mSv,降低约53%,差异有统计学意义(P<0.05),A组的对比剂碘量比B组降低约50%。结论:在自由心率时,容积扫描模式可获取高清、低剂量的左心房和肺静脉图像,对射频消融术前检查的房颤患者具有重要的临床价值。Purpose:To investigate the value of a 16 cm volume scanning mode in angiographic imaging of the left atrium and pulmonary vein in patients with atrial fibrillation(AF).Methods:Sixty AF patients who underwent CT angiography(CTA)examination before radiofrequency ablation were randomly divided into two groups:group A and group B.In group A,The 16 cm volume scanning mode with a tube voltage of 100 k V and automated tube current modulation was utilized.In group B,an ECG-gated spiral scanning mode was used,with all other scanning parameters identical to those in group A.The scan range for both groups extended from 2 cm above the tracheal carina to the diaphragmatic floor of the heart.Image quality,dose length product(DLP),and effective dose(ED)were statistically analyzed for both groups.Results:When heart rates were above 75 beats per minute,group A demonstrated significantly higher image quality than group B(P<0.05).At heart rates of 75 beats per minute or below,there was no significant difference in image quality between the groups.The radiation dose for group A was(4.01±2.82)m Sv,compared to(8.51±0.60)m Sv for group B,reflecting a reduction of approximately 53%in group A.Additionally,the contrast agent iodine dose used in group A was about 50%lower than that in group B.Conclusion:The 16 cm volume scanning mode can provide high-definition,low-dose images of the left atrium and pulmonary veins at variable heart rates,offering significant clinical value for pre-ablation assessment in AF patients.
