摘 要:目的本研究旨在探讨院前跨省市长途转运过程中,俯卧位通气对重症肺炎患者的应用效果,以期改善患者呼吸功能及整体预后。方法回顾性纳入了2023年4月至2024年5月期间于无锡市急救中心长途组接受跨省市长途转运的重症肺炎患者共计30例。患者在转运过程中依据病情被分为观察组(俯卧位通气组,15例)与对照组(常规仰卧位通气组,15例),对两组患者转运期间的氧合指数、血气分析结果以及不良事件发生率进行对比分析。结果俯卧位通气组患者在转运结束时的氧合指数显著高于对照组(P<0.05)。俯卧位通气组患者的不良事件发生率虽低于对照组,但差异无统计学意义(P>0.05)。结论在院前跨省市长途转运过程中,俯卧位通气能够显著改善重症肺炎患者的氧合情况,有助于稳定患者的呼吸功能,且安全性较高,值得在临床实践中推广应用。Objective This study aims to evaluate the effects of prone-position ventilation during long-distance intercity transport of critically ill pneumonia patients,with the goal of improving respiratory function and overall prognosis.Methods A total of 30 critically ill pneumonia patients who underwent long-distance intercity transport at Wuxi First Aid center from April 2023 to May 2024 were retrospectively included in the study.During the transportation process,patients were divided into an observation group(prone position ventilation group,15 cases)and a control group(conventional supine position ventilation group,15 cases)based on their condition.The oxygenation index,blood gas analysis results,and the incidence of adverse events during transport were compared between the two groups.Results The oxygenation index at the end of transport was significantly higher in the prone-position ventilation group than in the control group(P<0.05).Although the incidence of adverse events was lower in the prone-position ventilation group compared to the control group,the difference was not statistically significant(P>0.05).Conclusion Prone-position ventilation during long-distance intercity transport in the pre-hospital setting can significantly improve the oxygenation status of critically ill pneumonia patients,helping to stabilize respiratory function with a high safety profile.This approach is worthy of clinical adoption and further applic ation.
