磁共振HR-VWI在中枢神经系统血管炎诊断中的价值研究

摘  要:目的 探讨高分辨率磁共振血管壁成像(HRVWI)在中枢神经系统血管炎(CNSV)诊断中的价值。方法 回顾性选取2021年3月~2023年11月我院收治的60例CNSV患者为研究对象,入院后均接受磁共振成像(M RI)常规扫描及H R-VWI检查,观察患者临床症状、病变血管分布、病变管壁影像学特点,探究HR-VWI在CNSV诊断中的价值。结果 60例患者临床症状中,以感觉运动异常(61.67%)、头痛(50.00%)较常见,其次是行为认知异常(28.33%)和癫痫发作(21.67%),头晕(11.67%)、视力模糊(1.67%)及意识障碍(5.00%)较少见。60例患者共540支大、中血管,病变血管较多见于额叶(55.55%)、颞叶(38.33%)、基底节区(38.33%)。MRI常规扫描显示有89支(16.48%)大、中血管病变,病变血管管腔均有不同程度狭窄,HR-VWI检查显示有157支(29.07%)大、中血管病变,病变血管管壁呈向心性增厚伴强化,二者病变血管检出率差异有统计学意义(P<0.05)。结论HR-VWI可清晰且直观显示病变血管管壁情况,对提高CNSV诊断敏感度具有重要价值。Objective To explore the value of high-resolution magnetic resonance imaging vascular wall imaging(HR-VWI)in the diagnosis of central nervous system vasculitis(CNSV).Methods 60 patients with CNSV in our hospital from March 2021 to November 2023 were retrospectively selected as the research subjects.After admission,all patients underwent routine magnetic resonance imaging(MRI)scan and HR-VWI examination.The clinical symptoms,lesion vascular distribution and lesion wall imaging characteristics were observed,and the value of HR-VWI in the diagnosis of CNSV was explored.Results Among the clinical symptoms of 60 patients,sensorimotor abnormality(61.67%)and headache(50.00%)were more common,followed by abnormal behavior cognition(28.33%)and epileptic seizure(21.67%).Dizziness(11.67%),blurred vision(11.67%)and disturbance of consciousness(5.00%)were less common.There were 540 large and medium vessels in 60 patients,and the diseased vessels were more common in the frontal lobe(55.55%),temporal lobe(38.33%)and basal ganglia(38.33%).Conventional MRI scan showed 89(16.48%)large and medium-sized vascular lesions,with different degrees of stenosis in the diseased vascular lumen.HR-VWI examination revealed 157(29.07%)large and medium-sized vascular lesions,with concentric thickening and enhancement of the diseased vascular wall.There was a statistically significant difference in the detection rate of diseased blood vessels between the two groups(P<0.05).Conclusion HR-VWI can clearly and intuitively show the lesion vascular wall,and it is of great value in improving the diagnostic sensitivity of CNSV.
