摘 要:目的 探讨双源CT双能量成像技术对痛风性关节炎的临床诊断价值。方法 对122例关节炎患者的影像学和临床数据进行回顾性分析。比较血尿酸水平、常规CT影像检查和双源CT影像检查对痛风患者的检出情况。结果 双源CT双能量成像检测痛风性关节患者尿酸盐结晶的特异性、灵敏度、假阳/阴性率、阳/阴性预测值和诊断符合率分别为85.71%、87.04%、 14.29%和12.9 6%、97.92%和46.15%、86.89%,ROC曲线下面积是0.895。血尿酸正常患者尿酸盐结晶检出率低于高血尿酸患者,二者差异无统计学意义(P>0.05)。常规CT影像检查对关节积液和尿酸盐结晶不敏感,痛风患者检出效率低于双源CT双能量成像。结论 双源CT双能量成像技术在痛风性关节炎的临床诊断中具有较高的准确性和敏感性,相比于血尿酸水平和常规CT影像检查,其独特的能量成像能有效检测痛风性关节患者的尿酸盐结晶,为早期诊断和治疗提供可靠的影像学支持。Objective To investigate the clinical diagnostic value of dual-source CT dual-energy imaging for gouty a rthritis.Methods Imaging and clinical data of 122 patients with gouty arthritis were retrospectively analyzed.Blood uric acid level,conventional CT imaging and dual-source CT imaging were compared for the detection of gouty patients.Results The specificity,sensitivity,false positive/negative rate,diagnostic compliance rate,and positive/negative predictive value of urate crystals in patients with gouty joints analyzed by dual-source CT dual-energy gout post-processing software were 85.71%,87.04%,14.29%,and 12.96%,97.92%,46.15%,and 86.89%.respectively,and the ROC area under the curve was 0.895.Patients with normal blood uric acid had a higher urate Salt crystals were detected less frequently than in patients with high blood uric acid,and the difference was not statistically significa nt(P>0.05).Conventional CT imaging was not sensitive to joint effusion and urate crystals,and the detection efficiency was lower than that of dual-source CT dual-energy imaging.Conclusion Dualsource CT single-energy imaging has high accuracy and sensitivity in the clinical diagnosis of gouty arthritis,and compared with blood uric acid level and conventional CT imaging,its unique energy imaging cha racteristics make it effective in detecting urate crystals in patients with gouty joints,which provides reliable imaging support for early diagn osis and treatment.
